Accident type, without injuries

SV805 Lube Truck was traveling towards the Porter Pit to service SH004 when unit lost power in cab, operator safely brought the truck to a complete stop and set park brake. Operator then looked out the window to the right seen haul truck passing then looked left and saw a red glow coming out of the articulation.

An unplanned sulfur fire occurred on the 4500 level of the C4 south wall at 8:45 pm. No equipment, no structures and no miners were put in harm.

An unplanned sulfur fire occurred on the 4450 level of the C4 south wall at 6:30 am. No equipment, no structures and no miners were put in harm. There was an existing sprinkler system in the area cooling hot spots. MSHA issued a 103(j) order at 7:40 am to prevent destruction of evidence. Sprinkler system extinguished fire at 8:25 am.

An unplanned sulfur fire occurred on the 4550 level of the C4 south wall at 7:20 pm. No equipment, no structures and no miners were put in harm. An access ramp was pushed in with dozer and sprinkler connection was completed.

An unplanned sulfur fire occurred on the 4550 level of the C4 south wall at 5:30 pm. No equipment, no structures and no miners were put in harm. Operators extinguished the fire at 5:44 pm.

An unplanned sulfur fire occurred on the 4500 level of the C4 south wall after a blast at 4:50 pm. No equipment, no structures and no miners were put in harm. Operators extinguished the fire at 6:50 pm.

A fire was reported to MSHA 01-05-15 at approximately 10:40 pm in the pit. MSHA issued a 103(j)order to prevent the destruction of evidence. Upon MSHA inspection at approximately 1:00 am there was a small hotspot on the south highwall.

A water truck inadvertently struck a portable toilet which was occupied. The occupant of the portable toilet suffered fatal injuries.

While loading drip lines onto a fork lift the employee states wind blew dust under his glasses and into his eye.

While closing clarifier door, employee reached up move lever into lock position, his finger became pinched between locking arm and clarifier.

On 1-27-15 Mine Operator reported a back injury occurred at mine while being loaded by the electric shovel. Under investigation, pending results.

Over-exertion in pulling or pushing objects

EE tried to pull a long length of 1"" rubber hose, stained his back and then asked for help moving the hose, moments later 2 more EE's arrived to help move hose, after hose was moved the EE felt pain in his back and reported the injury to his supervisor they filled out a C-1 report. EE did not want medical help at this time, worker had pain next morning, taken to hospital for exam.

Over-exertion in lifting objects

While cleaning, the worker lifted a bag of household type garbage, while simultaneously twisting in order to carry it out of the office resulting in a lower lumbar strain.

Over-exertion (Not Elsewhere Classified)

Worker completed pre-shift equipment inspection on Unit #408 (CAT D11R Dozer). The worker began the process of ""climbing onto the machine"" in order to access the cab by reaching for the grab handle with his right arm. The workers foot shifted, causing tension to his extended right arm resulting in a dislocated, right shoulder.

Fall to the walkway or working surface

The worker removed an 18""x18"" grate near the sump in order to help facilitate cleaning of the process room. He stepped over the opening in order to help another worker with a hose. After helping, he stepped back with his right leg, which went into the opening. He slipped onto his right side, resulting in a bruise/strain to his right rib cage area.

Over-exertion in wielding or throwing objects

Swinging a 14 lb hammer and pulled a muscle in his left shoulder.
