Accident type, without injuries

Roof fall occur at a 4-way intersection in No. 3 entry near spad No. 55, 1150 outby pillar line. Water seepage is present and the fall was 50 ft. long by 20 ft. wide and 6 ft. thick.

Roof fall 9,000 feet outby section in upper seam, old works on tunnel belt, neutral entry No. 2 at Spad 1106, 1st. East Mains. Will not be rehabilitated.

Roof fall occured in a 4-way intersection in No. 2 entry at spad 32, 2200 feet from face and 600 feet from portal. Water is seeping thru roof and fall was 70' x 20' x 6 to 8 feet thick.

A fall occured 300 feet outby the face, 60 feet outby spad No. 249 in No. 6 entry at a 4-way intersection. The roof has multiple cracks and prior to the fall, timbers and heitzmann jacks had been installed. The fall was 60 ft. long, 20 ft. wide and 6-8 ft. thick.

A roof fall occured in the #6 entry at # 9 break, 1 break inby spad 2256 of the #2 Left Mains. The fall measured 20 ft. x 18 ft. x 4.5 to 5 ft. thick.

Roof fall in No. 6 entry between Spad No. 58 and No. 64, fall is 30'-40' long x 18' wide x 8'-10' thick. Fall is in intake entry and air will be re-routed into No. 5 entry. Reflectors and danger tags are hung. Area will be breakered off with 100 ton Heintzmann, Pizza Jacks.

Roof fall in 2-A Tunnel Belts, #4 Entry at Spad #72, neutral entry. Fall was 50'x20'x5'.

Fall occur between a 3-way and 4-way intersection. The 4-way intersection was supported with 100 ton Heintzmann jacks and I-Beams with 4-100 ton pizza pan jacks on the side where the fall occured. Fall is 30 ft. long, 19 ft. wide by 5-6 ft. thick

Fall occured in a four-way intersection near spad No. 19. Fall is 30 ft. long by 18 ft. wide and 6-7 ft. thick. Area was recently mined. Multiple cracks and water are present. Cracks had been strapped with T-3 roof channel with 48 in. resin roof bolts.

Roof fall in 1 North Mains, #2 Entry at first crosscut underground approximately 20' x 25' x 6' thick. Area was dangered off breakers being set.

Accident type, without injuries

Roof fall occurred in No. 4 entry 150 ft. inby spad No. 1511, 400ft. from active face. The roof fall is 60 ft. long, 20 ft. wide and 6 to 8 ft. thick. All entries are breakered off. The roof is laminated and was bolted using No. 5 x by 48 in. resin grouted re-bar.

Accident type, without injuries

The roof fall occured in No. 2 entry of South Mains near spad No. 87, 1900 feet from active face. This area was developed in March 2001. The fall measures 50 feet long by 20 feet wide and 6 foot thick. All entrances to the fall are breakered off.

Over-exertion in wielding or throwing objects

Employee injured back while driving slope belt drive bull gear onto shaft with sledge hammer. Note: This accident was not ruled compensable until 3/7/06, received notification from Brick Street on 3/9/06. Originally beleived accident was not work related since first day missed was 1/26/06.

Struck by falling object

Loose coal mashed employee's foot against the dust box of the roof bolter, breaking his left ankle.

Over-exertion in pulling or pushing objects

EE stepped in a hole while pulling roof bolter cable slack and ALLEDGEDLY injured his back. His first Doctor vist was on 4/18/06, missed work on 4/19/06 and returned to work on 4/25/06. We do not feel this a a legitimate lost time injury.

Over-exertion in lifting objects

EE, eve shift roof bolter operator, was attempting to move miner cable when he apparently strained muscle in low back.

Over-exertion in pulling or pushing objects

Miner oprator bent over to pull miner cable and apparently strained muscle in lower back.

Over-exertion (Not Elsewhere Classified)

Employee was blocking the feeder. His foot got caught under the turnbuckle on the tailpiece and when he moved he twisted his knee.

Over-exertion in lifting objects

Employee was loading a scoop with belt structure when he felt pain in his back. He continued to work thru 11/8/05.

Fall to the walkway or working surface

EE was prying a rock out of belt conveyor head piece, slate bar slipped and he fell straining his back.

Fall onto or against objects

Employee was geting out of the scoop to close the airlock doors. He slipped in the mud with one foot in the scoop and one foot out. When he fell he hit his groin area on the scoop frame.

Fall to the walkway or working surface

Employee was standing on bolter cable while it was tramming. Bolter turned and jerked cable and flipped him, hitting head. He continued working that shift and all scheduled shifts until he reported to work on 9/8/05 and told supervisor his head hurt and was dizzy and went home before shift started. This injury is being CONTESTED since he worked 2 weeks before having a problem.

Over-exertion in lifting objects

EE was setting a steel roof jack under a steel I-beam at the fan housing. Picked up jack & twisted his left knee & strained his shoulder. He returned to work on 8/22/05 but was unable to return to work on 8/23/05. EE HAD ROTATOR CUFF & KNEE SURGERY.

Struck by falling object

EE was operating roof bolter when a kettle bottom fell on canopy and slid off striking his right shoulder.

EE was moving and relocating roof bolter and canopy hit his left shoulder and neck. Worked remainder of shift and 2 additional days; last day worked 7/25/05.

Struck by falling object

EE alleges that a rock fell from roof, striking his left ankle. It was also reported that it was a rib roll that hit his left ankle. A coworker was reported as a witness but informed management that he did not witness the injury. EE finished working his regular shift.

Over-exertion in lifting objects

Employee ""ALLEGED"" that he was picking up a bag of rock dust at the #1 Airlock Doors and pulled something in his back. There were no witnesses to this injury.

Struck by falling object

While near the roof bolting machine, a piece of rock approx. 4"" x 5"" x 2-1/2"" in size fell, striking EE's lower back.

Employee was changing feeder flight when the flight slipped and smashed finger.

Employee received a broken right leg when hit by a rib roll.

EE was running the roofbolter when coal rib rolled out from behind and pushed him into the roof bolter.Left leg hit panic bar and right shoulder hit canopy post.

Employee was using a torque wrench on a roof bolt when he felt a pain in the middle of his back. First day missed work was 3/8/05.

EE was hit on head and shoulder by piece of rock while bolting roof.

EE was moving and pulling miner cable and strained groin area. Worked scheduled days until 2/25/05, first day missed.

EE hit head on half-header; he fell down on his right arm.

Struck against stationary object

Employee hit left knee on belt head motor plate while working on #4B belt head.

Struck by rolling or sliding object

Employee came to roof bolter to bolt in place of regular operator who left sick. He started to install first row of bolts, when rib rolled striking injured's left side.

Employee was bolting top when a piece of rock fell striking his left leg.

Employee was drilling hole for roof bolt using 2 piece steel. Finished drilling with first piece and dropped head down and added second piece. Started drill pot up to connect steel and drill pot got hung or stuck leaning steel out towards operator, then came loose and steel smacked right thumb while holding first piece of steel.

Employee got out of the shuttle car and was starting to return to his seat when he experienced pain in his left knee.

EE was rolling up welding cable, when cable got tangled up on a mantrip. Went to untangle cable from mantrip, walked by an open welding rod container and scrapped right hand against container.

EE hit his left hand with hammer while working on #6 belt head, requiring 5 stitches in palm of hand.

EE was installing an idler in a mobile roof support, the idler rolled back and broke his left ring finger.

Struck by falling object

EE, evening shift scoop operator, received contusion and laceration of right hand when a rock fell from the roof and struck EE.

EE. filed an ALLEGED claim for Occupational Pneumoniosis on 12/29/2005. Received notification on 5/4/2006 of Claim No. 2006022866. His last day worked was 12/15/2004 and retired on 7/1/2005.

EE filed an ALLEGED claim for Occupational Pneumoniosis on 10/31/05. His last day worked for Long Branch Energy was 5/18/05, due to other illness. Received notice of Claim No. 2006019204-OP on 1/23/06.

Employee filed an ALLEGED claim for Occupational Pneumoniosis on 9/19/05 after working 1 yr. 10 mo. for Long Branch Energy. Received notice on 9/21/05, Claim No. 2006010288

Employee filed an ALLEDGED occupational pneumoniosis claim on 6/3/05, Claim No. 2005044352-OP.

Employee filed an ""ALLEGED"" occupational pneumoniosis claim No. 2005030425 on 2/9/05. Received notification on 2/23/05.

Employee filed an ALLEGED claim for Occupational Pneumoconiosis on 09/22/05. Received notification on 10/11/05. Claim No. 2006010933-OP. On 3/9/06 the OP Medical Board findings of 1/12/06 did not make a diagnosis of OP. No award was granted and is not entitled to any medical benefits. Therefore ee should be removed from the OP list.

Inhalation of radiations, caustics, toxic and noxious substances

Employee filed an ALLEGED claim for Occupational Pneumoconiosis on 09/19/05. Received notification on 10/11/05. Claim No. 2006010290-OP. On 3/9/06 the OP Medical Board made a diagnosis of OP without pulmonary function impairment. No award was granted & is not entitled to any medical benefits. Therefore ee should be removed from the OP list.
