Accident type, without injuries

A 40' x 20' x 5' high roof fall occured in the #6 entry intersection of the Old Cazy section. The area will be breakered off

A 25' x 20 x 8' thick roof fall occurred in No. 2 entry, one break inby spad No. 15846. The area will be breakered off and air will be rerouted into the no. 1 entry.

A 10' x 19' x 6' high roof fall occurred in No. 1 entry approximately 150 feet inby the return air shaft, 3200 feet from No. 1 Section. Roof is supported with 48"" resin bolts. 2 cribs were installed prior to the fall. All entries to the fall are breakered off.

A 20'x20'x4' roof fall occurred in the intersection of 4 right off 2 west mains in No. 3 entry at spad No. 13919. The fall has been dangered off and breakers set. The fall will not be cleaned up.

On Sept 24, 2007 a roof fall was found near spad #739 along #7 belt in the #7 entry. The fall measured approx. 74' long x 20' wide x 7' thick. The fall does not impede airways or travel and will be timbered off to prevent travel.

A fault and rotten top caused a fall to occur on #8 belt near spad 4394. The fall was approximately 130 ft. by 19 ft. by 5 to 6 ft. thick.

Roof fall occured in the Spruce Laurel Mains, Sycamore Section, No. 5 Entry 1 break inby spad No. 1365. Fall was 60 ft. long by 20 ft. wide and 8 ft. thick. Set jacks at fall and will be moving stopping to reroute air and lifeline.

Roof fall occured in a 4-way intersection at 001 section, No. 5 neutral entry near spad No. 24. Fall was 20' wide by 20' long and 6' to 8' thick. Area was developed in March 2005 and was bolted with No. 5, 48"" full grout bolts.

A roof fall occurred on old 2A belt line between 3 and 4 head, 12,500 feet from face. Fall was 60' to 70' long by 5' to 20' wide. Area will be timbered off.

Fall 40' x 20' x 4' occur in the main intake escapeway 14 breaks from fan and 11,000 feet from face. Withdrew men from mine and reported fall to agencies.

Roof fall occured in No. 2 entry near spad No. 725, 800 feet outby active face. Rock is 90 ft. long x 20 ft. wide and 6 to 8 feet thick. Area had some roof cracks with water seepage. Roof was bolted with full grout No. 5 x 48 in. bolts. Ribs had been timbered on 4 ft. centers.

Fall occurred on pillar line, approximately 70 ft. long x 70 ft. wide x 4-6 feet thick. Fall covered front of 14CM9 Joy Continuous Miner.

Caught in, under or between a moving and a stationary object

While cleaning the off-side walkway on #4 belt, 2 Heintzmann jacks became dislodged and a large rock struck employee pushing him into the belt line. The accident is currently under investigation by the WVOMHS&T, MSHA and the Company.

Struck by rolling or sliding object

EE's lower right leg was bruised by a rib roll.

Fall to the walkway or working surface

Employee ""ALLEDGED"" to have fallen over the miner cable that was next to the roof bolter and injured his right shoulder, arm and wrist. This alledged incident is still under investigation.

Employee was rock dusting using 50# bags of dust. He stepped on uneven bottom twisting left knee. He continued working until 1/4/08, his first day missed.

Struck by flying object

Employee was shearing rib for belt line with miner, rock came out of conveyor chain hitting him in bottom of chin.

Employee was getting off the roof bolting machine, stepped in hole and twisted his right ankle.

EE tripped and fell into rib, caught weight on right hand/arm.

Employee was leveling belt structure. When he lifted bottom stand he felt a burning sensation in his back. First day missed due to injury was 9/26/07.

Employee was removing shutle car cable reel shield with a slate bar. The slate bar slipped and he injured his right arm and shoulder when he caught himself as he was falling. First day missed work due to injury was 11/1/07.

Employee was starting to drill a hole for a roof bolt, his right hand was on the drill steel when a piece of rock fell from roof striking him on the right middle finger, cutting it at the first joint.
