A 40' x 20' x 5' high roof fall occured in the #6 entry intersection of the Old Cazy section. The area will be breakered off

A 25' x 20 x 8' thick roof fall occurred in No. 2 entry, one break inby spad No. 15846. The area will be breakered off and air will be rerouted into the no. 1 entry.

A 10' x 19' x 6' high roof fall occurred in No. 1 entry approximately 150 feet inby the return air shaft, 3200 feet from No. 1 Section. Roof is supported with 48"" resin bolts. 2 cribs were installed prior to the fall. All entries to the fall are breakered off.

A 20'x20'x4' roof fall occurred in the intersection of 4 right off 2 west mains in No. 3 entry at spad No. 13919. The fall has been dangered off and breakers set. The fall will not be cleaned up.

Roof fall in #7 intake entry of Spruce Laurel Mains, 3 breaks inby spad #13367, 25' x 20' x 5' thick.

On Sept 24, 2007 a roof fall was found near spad #739 along #7 belt in the #7 entry. The fall measured approx. 74' long x 20' wide x 7' thick. The fall does not impede airways or travel and will be timbered off to prevent travel.

Fall occured in the intake entry #2, 14000 feet away from active face. Will clean up walkway and set jacks for escapeway. Fall was 12 ft. by 16 ft. by 3 to 4 ft. thick.

A roof fall measuring 25 ft. by 20 ft. by 5 ft. thick occurred in the #5 neutral entry across from spad no. 97.

A fault and rotten top caused a fall to occur on #8 belt near spad 4394. The fall was approximately 130 ft. by 19 ft. by 5 to 6 ft. thick.

Roof fall approximately 50 ft. x 20 ft. x 6 to 8 ft. thick occur in the No. 5 intake entry, 90 ft. inby spad No. 13622. Area is dangered off, lifeline and stopping line will be moved/re-routed.

Roof fell in No. 3 entry at intersect 2 breaks outby spad 15918. Fall was 20 ft. x 20 ft. 5 ft. thick.

Roof fall occurred in #2 entry at spad # 105. Fall was 17' x 40' x 4' thick.

Roof fall occured in the No. 5 entry of 2 left mains, 1 break outby spad No. 2249 in an area under construction being rehabilated for the section relocation. The fall was approximately 60 ft. by 20 ft. by 4ft. thick.

A roof fall , approximately 40 ft. y 20 ft. by 4 ft., occured about 8,500 ft. outby section at spad No. 86.

A roof fall, approx. 60ft. x 20 ft. x 6 ft. thick occured in the No. 6 entry 2 breaks outby spad No. 121, approx. 8000ft outby the section. The fall has been dangered off and breakered off.

Roof fell between No. 1 and No. 2 entry inby spad No. 13640. Fall was 50' long by 18' wide by 8' thick.

Roof fall in Winifrede No 15 mine occurred 3 breaks underground at intersection from No. 4 entry to No. 3 entry, 1 break inby spad No. 21. Fall was 70 ft. long by 6 ft. wide and 5 ft. thick.

Roof fall occured in the Spruce Laurel Mains, Sycamore Section, No. 5 Entry 1 break inby spad No. 1365. Fall was 60 ft. long by 20 ft. wide and 8 ft. thick. Set jacks at fall and will be moving stopping to reroute air and lifeline.

Roof fall occured in a 4-way intersection at 001 section, No. 5 neutral entry near spad No. 24. Fall was 20' wide by 20' long and 6' to 8' thick. Area was developed in March 2005 and was bolted with No. 5, 48"" full grout bolts.

A roof fall occurred on old 2A belt line between 3 and 4 head, 12,500 feet from face. Fall was 60' to 70' long by 5' to 20' wide. Area will be timbered off.

Fall 40' x 20' x 4' occur in the main intake escapeway 14 breaks from fan and 11,000 feet from face. Withdrew men from mine and reported fall to agencies.

Roof fall occured in No. 2 entry near spad No. 725, 800 feet outby active face. Rock is 90 ft. long x 20 ft. wide and 6 to 8 feet thick. Area had some roof cracks with water seepage. Roof was bolted with full grout No. 5 x 48 in. bolts. Ribs had been timbered on 4 ft. centers.

Fall occurred on pillar line, approximately 70 ft. long x 70 ft. wide x 4-6 feet thick. Fall covered front of 14CM9 Joy Continuous Miner.

EE's lower right leg was bruised by a rib roll.

Employee ""ALLEDGED"" to have fallen over the miner cable that was next to the roof bolter and injured his right shoulder, arm and wrist. This alledged incident is still under investigation.

Employee was rock dusting using 50# bags of dust. He stepped on uneven bottom twisting left knee. He continued working until 1/4/08, his first day missed.

Employee was shearing rib for belt line with miner, rock came out of conveyor chain hitting him in bottom of chin.

Employee was getting off the roof bolting machine, stepped in hole and twisted his right ankle.

EE tripped and fell into rib, caught weight on right hand/arm.

EE was getting out of roof bolting machine, slipped and strained lower back.

Employee was leveling belt structure. When he lifted bottom stand he felt a burning sensation in his back. First day missed due to injury was 9/26/07.

Employee was helping with miner cable when rib rolled off striking him in the side.

EE was making repair to continuous miner, rib rolled striking his left knee.

EE was making repair to continuous miner, rib rolled striking his right leg.

Employee was bolting roof when a rock fell from the top striking him on the shoulder and right arm.

EE was making a cable splice and struck left hand with a hammer.

Employee ""ALLEGEDLY"" injured his neck and shoulder when belt was being changed out. Belt structure came apart and fell on him while sitting in scoop. He finished the shift.

Employee was stacking belt structure when a piece slipped out of hand and struck his left knee.

Employee was standing in front of miner cable, miner pulled forward and rt. foot got tangled up in cable.

Employee ""ALLEDGEDLY"" twisted his lower back while loading belt structure in scoop bucket. He did not miss work until 4/25/07.

Employee injured his left foot and ankle when his foot tangled in the miner cable while running from a fall on the pillar section.

EE was pulling belt back, stepped in a hole and twisted right knee

Employee was standing next to the scoop bucket when the scoop operator lowered the scoop bucket on his foot.

Employee was operating roof bolter when a piece of rock fell from the roof, strking his right hand.

EE was bolting roof when a piece of rock fell and hit his right hand and forearm.

Employee was helping on continuous miner when a rock 3"" thick by 2' by 4' fell and pushed him to the floor.

EE was setting drill steel when he hit wrong lever, closing drill steel holder clamp onto his finger.

EE ran over a rock while running a scoop and jarred his whole body, alledged to have hurt lower back.

Employee's glove got caught on the roof bolter drill steel, twisting his right hand. His last day worked was 4/15/08 and started missing work on 4/16/08 for hand surgery.

Employee stepped on gob, twisted right knee.

Employee was removing shutle car cable reel shield with a slate bar. The slate bar slipped and he injured his right arm and shoulder when he caught himself as he was falling. First day missed work due to injury was 11/1/07.

Employee was starting to drill a hole for a roof bolt, his right hand was on the drill steel when a piece of rock fell from roof striking him on the right middle finger, cutting it at the first joint.

Employee was drilling a hole for a roof bolt; rock fell from roof and landed on his left forearm.

Employee was drilling a hole to install roof bolt when a piece of rock fell striking his right forearm.

Employee was helping change shield on continuous miner, caught right hand under shield and caused a laceration.

Employee was making a cable splice when the knife slipped and cut his thumb.

Employee was cutting a piece of belt when belt knife cut through the belt and struck his leg.

Employee was cutting a fly pad with belt knife, slipped and cut left forearm.

Employee was cutting a piece of belt with a belt knife when it slipped and cut his leg. The cut required 4 stitches.
