Accident type, without injuries

A roof fall occurred in #1 entry, 2 breaks outby spad #255 and 7500 feet from the active face. The fall was approximately 25' x 20' x 4.5' thick and will not be cleaned up at the present time.

A 25' x 30' x 16' high roof fall occurred in 1st. left mains in #3 entry at spad 2383 approx. 4200' from face. The roof fall area was dangered off with caution tape.

A roof fall occurred in the #1 return entry, 100 feet outby spad # 3281 and 4500 feet from the active section. The fall was 20' x 12' x 2' to 4' thick.

Roof fall occured in #5 entry in the top seam, 3 breaks inby temporary section. Area will be timbered off, bolted, set cribs and clean-up the fall and bolt on cycle.

The fall occured in the #3 entry approximately 1600 feet from the surface on 1-A belt roadway. Fall is about 40' x 20' x 2 to 4 feet thick in a remote neutral area and will be timbered off.

A 3' x 3' x 8' deep void occurred when floor fell into the lower seam at no. 98 crosscut in no. 3 entry. The air is ingassing with 0.0% Methane and 20.7% Oxygen readings.

A roof fall measuring approximately 20' x 30' x 6' high occurred at break 25 in No. 5 neutral entry, 15,000 ft. from the face.

A 25' x 15' x 1.5' high roof fall occurred in No. 2 entry of 1 West Mains, 4 breaks inby spad No. 84.

Roof fell between #1 & #2 Entry near spad # 13640 in the Spruce Laurel Mains. The fall was 30' x 20' x 5' thick and will not be cleaned up. Will set cribs, build new stoppings and re-route life line.

A roof fall, 20'x20'x5' thick, occurred in the No. 1 entry of the Cazy mains 2 breaks inby spad 13050. The area has been breakered off.

A 15'x20'x4' high roof fall occurred in the #2 belt entry approximately 10,000 feet from the active face.

Fall to the walkway or working surface

Employee was making routine examination of No. 6C Sycamore Seal, he slipped and fell fracturing his left arm/wrist.

Struck by falling object

EE was operating a roof bolter when a rock fell from roof, striking his left ring finger.

Over-exertion in lifting objects

Employee was helping make a belt move, picked up a piece of structure and twisted lower back.

Struck against a moving object

Employee was operating a shuttle car when he hit a hole with one tire. The jolt bounced him out of his seat and jammed his head into the canopy.

Struck against stationary object

Employee was walking thru a fly curtain and hit his head on a cinder block that remained stuck to the rib on a pillar section. This mine is on an alternate work schedule, 4 ten hour shifts a week and this is the ninth day the employee has missed work.

Struck by falling object

Employee was walking on the section; a rock (approximately 1-2"" thick by 24"" round) fell from the roof hitting his hard hat and right shoulder.

Over-exertion in lifting objects

Employee was lifting shields off of miner, Injured/strained lower back. First day missed due to injury was 8/12/08.

Struck by falling object

EE was putting up a roof bolt, when a piece of rock fell from the top and knocked him against the roof bolter.

EE stepped off the back of the roof bolter, into a hole, twisting his left ankle.

Employee was spot bolting top in an outby area. A rock 23"" x 26"" x 6"" thick fell from the rib hitting him in the back.

Employee was hauling a load, when his scoop ran over some draw rock and jammed his head in the top.

Employee was walking in a low top area; he ""ALLEGEDLY"" turned his back when he raised up in a higher top area. Mine management was not aware of the injury until he was scheduled to return to work after the vacation period on 7/8/08.

EE pulled something in his lower back while shoveling mud from under section tail piece onto belt.

Employee was installing rib bolts. When removing steel the rib rolled, striking steel causing steel to strike left shoulder and ribs.

Caught in, under or between a moving and a stationary object

EE on #1 belt was working with another EE, the victum was in a pinch point area and was struck by battery operated 4-wheeler, pinning EE against Rib and mantrip.

Employee was using a slate bar when it slipped and allegedly struck him in the ribs.

Employee was pulling on conveyor belt when he injured his back.

Employee was shoveling under the belt conveyor when he heard a pop in his right arm. He continued working till his first day missed on 5/12/08.

EE was bolting the rib and the rib rolled out, striking his right leg and ankle.

EE strained his back when lifting on the miner cable during a power move. He continued working his regular job until 3/18/08, his first day missed.

Employee was inserting a five foot roof bolt with plate. The plate slid down the bolt striking his left hand and thumb, possible fracture.

Employee was waiting for his shuttle car to be loaded when a foreign object hit him in the eye.

Employee was driving 4-wheeler past a timber and his hand was caught between the timber and his ride.

Employee was stacking structure into a supply car and a piece fell and cut his finger.

Employee was changing a belt scraper; scraper fell catching his left hand between scraper and belt frame.

Employee was pulling a ventilation curtain. He looked behind to observe the curtain and ran into an overhanging brow.

Injury ""ALLEDGED"" to have injured his back when he hit his head on the mine roof while loading 8 foot roof bolts onto the roof bolting machine. He continued to work until his first day missed on 6/14/08.

Fall onto or against objects

Employee was changing batteries on a scoop and fell onto the scoop fender and injured his tail bone. Employee started missing 1/10/08.

EE was helping make a belt move, he stepped on a crossbar, it flew up striking his right arm and left cheek of his face.

At #9 belt head on #2 section strained right knee while changing reducer. Note: EE didn't miss any work while at Mine #25. He transferred to Mine #18 on March 6, 2008.
