Mechanic was manually aligning a track pin up for installation and experienced a pulling sensation in employee's bicep when the pin was driven into place.
Employee was operating a scraper when EE felt a pain in the back.
The pin on the slot wrench broke while making a connection. The driller asked the helper to move the wrench back into place by hand. After doing so the driller placed hand back on the controls to move the hydraulic ram. Then the helper moved hand back onto the wrench as the ram moved, pinching EE's finger.
Employee complained of pain in employee's left rear during employee's shift operating haul truck.
Employee was conducting pre-shift exam on shovel after maintenance had been performed and employee stepped on a lifting eye located on top of engine compartment, fell hurting right ankle.
Employee was installing an intake boot when they felt a pain in right elbow.
Employee was in the process of changing ground engaging tools on a road grader when employee felt a sting to employee's right forearm.
Employee sprained right ankle while pulling cable in pit. Employee sat down in a pickup and appeared to lose consciousness due to pain. Employee was given restricted duty and was placed in a walking boot.
EE was in van incident that occurred on 10-31-17. EE was evaluated and given full release that same day. On 11-2-17, ee was taken for further evaluation and given another full release. On 11-09-17 employee went to chiropractor for hip re-alignment and was again given full release after treatment. Employee reported chiropractor visit to Marigold Mine on 11-20-17
While lifting GET for scraper with overhead crane and magnet, the GET slid across hand cutting pinky finger.
After lifting the hammer from the interchange, employee was setting the hammer down to lean on the daybed. Employee slid the hammer down left forearm and a shard of metal off the hammer inserted into left forearm requiring stitches.
