Struck by falling object

While off loading a trailer a co-worker pulled on a pallet, containing sacks of aprox. 300lbs., with a pallet jack. This caused another pallet to shift resulting in a top sack falling onto the injured workers back.

While attempting to remove the table hammer the employee raised the hydraulic wrench with right hand and reached for the hammer with left hand and the hydraulic wrench slipped out of EE's hand and impacted the left hand.

Struck against a moving object

Large rock struck haul truck bed while being loaded by electric shovel, jarring haul truck operator.

Over-exertion in lifting objects

Employee was digging in frozen drill cuttings for a sample when experienced pain in neck and back. Reported to supervisor on shift and requested medical evaluation.

Fall from machine

Employee was working on engine, slipped on intake pipe and fell

Struck against a moving object

Big rock fell from the shovel bucket and landed on dove tail of haul truck. Bouncing the truck.

Over-exertion in pulling or pushing objects

Employee bent down to open the door on connex and when employee pulled on the door felt a pain in middle of back.

While placing pipe on a dozer at leach pad, the ground underneath broke through and twisted ankle.

Employee reported that incident occurred on June 25th, 2018 and supervisor was sent a text message at end of shift while on crew bus going home. Employer was notified on July 31, 2018 with a work limitation from personal physician. While pulling self up drill step, employee felt a pop in right shoulder.

Fall onto or against objects

While walking to access the cab of rubber tired dozer, employee lost footing/slipped on the decking which resulted in the employee falling into the front hand rail of the machine next to the cab. The employee's head and ribs made contact with the front handrail

Over-exertion in lifting objects

Employee was lifting a 4x4 board and felt a pop in back

Fall from machine

Drill operator slipped while climbing down boarding ladder striking right knee on ground.

While disconnecting a 2"" cam-lock hose connection the employee was having trouble getting the hoses to separate. The employee put one hose between EE's legs and when the hoses came apart the cam-lock hit the employee in the mouth chipping EE's tooth

Struck by flying object

Injured employee was disassembling metal tracks. While using a hammer to remove metal pins, a small metal piece of metal flew off the pin and hit the employee in the upper lip. The employee was wearing all face shields at the time. Employee did go to the local clinic and received 4 stitches. Employee was able to go back to work that afternoon and was released to full duty.

While working at lime silo, employee slipped on ice.

Drill operator was pulling samples with sample catcher when the operator felt pain in lower back.

Contact with heat

Working on the slope of leach pad walking up and down for about 3 hours. Employee felt dizzy and nauseous.
