Struck against a moving object

Metal on metal between 2 haul trucks traveling on haul road. Fatigue was the contributing factor.

Fall from machine

Employee slipped off a dozer while exiting the equipment.

Caught in, under or between a moving and a stationary object

Employee was removing haul truck tires using a rad gun. When employee began to remove a nut on haul truck tire, the rad gun was not set to remove causing rad gun to tighten and pinch finger between nut and rad gun.

Fall to the walkway or working surface

Employee was moving tables in the ambulance bay and tripped on the ambulance stop blocks.

Struck by flying object

While the employee was removing nuts from rubber tire dozer cutting edges, the air exhaust from the one-inch impact gun blew dust and other materials towards the employee. The employee felt something in eye but continued to work on 12/19 and 12/20. On 12/21 the eye was red and irritated and the employee reported it to the supervisor.
