Accident type, without injuries

A roof fall occurred on the 2L4W section, on the 3C panel, at cross cut 11, entry #5. The cause of the fall was weak laminated sandy shale commonly referred to as stack rock.

The elevator at our Hart street portal failed to operate properly. After inspection and cleaning procedures the brakes on the elevator would not hold the cage in one position at the top and bottom landings. It was determined that carbon blown off the motor armature was deposited on the brake pads of the elevator causing them to slip.

A roof fall occurred in #1 entry of the 2nd mine at survey station 41+50. The fall measured 110' long x 18' wide x 9' thick. The area had been supported with 6' grouted and 9' double lock bolts. All approaches to this area have been timbered off.

A roof fall occurred between #3 and #4 entry of the 2nd MNE at survey station 1 and 40. The fall measured 40' long by 18' wide by 10' thick. This area had been bolted with 9' double lock and 6' fully grouted resin bolts. All approaches have been timbered.

A roof fall occurred on the 2L4W panel on the 3B belt at cross-cut 18 in entry #4. The cause of the fall was weak laminated sandy shale, commonly referred to as stack rock. The fall covered the 3B belt for a distance of about 30 feet. The fall was cleaned up the area re-supported and timbered.

A roof fall occurred on the 2L4W-2L panel at cross-cut 18 in entry #6. The cause of the fall was a weak laminated sandy shale. This fall was in an adjacent entry next to the primary escapeway for our #3 unit.

A roof fall occurred on the 1L4W section at cross-cut 14 from entry 3 to 5. A slick sided slip fell on the miner while mining entry 4-5 blocking retreat of the miner. This was at the end of this crews shift. Before recovery work for the miner could begin the roof continued to fall and fell from #4 entry over to #3 entry.

The Hart Street elevator malfunctioned and would not operate properly. A door switch malfunctioned and would not let the elevator function. The door switch was replaced and the elevator was returned to normal operations around 12:30 A.M.

A roof fall occurred in entry #1 of the 2nd N.E. mains at survey station 12+60. The fall measured 140' long x 19' wide x 9"" thick. This area had been bolted with 6' fully grouted and 9' double lock bolts. All approaches will be timbered.

The slope hoist at our Weatland Portal failed to operate properly. The investigation revealed that the slack rope switch located near the hoist drum had been tripped causing the malfunction. The switch was reset and the hoist was returned to operation.

A roof fall occurred on the 3W roadway at cross-cut 5 in entry #5. The cause of the fall was weak laminted roof shale. The fall blocked a supply road to the Wheatland portal. The fall will be cleaned up and the roof and area will be re-supported.

The slope hoist at our Wheatland portal malfunctioned. It would only travel up and would not lower into the mine. It was found that a safety switch located near the hoist drum had been tripped which caused the malfunction. The switch was reset and the hoist was returned to operation.

The Hart St. portal elevator malfunctioned with one of our emplyees on it. A brake release contactor failed when the elevator was 20 feet from the top landing. This caused the brakes to automatically lock on the elevator. The contactor was replaced and the elevator was returned to service at 1:30 p.m.

A roof fall occurred at x-cut 127-128 in entry #4 of the 3W mains. The fall covered about 20' of belt stucture. The fall was 20' long, 15' wide and 4' high. The cause of the fall was a weak laminated sandstone and shale mix.

The emergency hoist malfunctioned and would not lower the escape capsule into the mine. An electric motor brake locked up and would not allow the cage to lower. The brake was repaired and the hoist returned to service.

A roof fall occurred on the 2L4W panel in entry #7 at Cross-cut 32-33. The cause of the fall was weak laminated roof strata. This entry has additional supports in it and had been dropped from the mining cycle.

The slope hoist at the Wheatland portal malfunctioned. The hoist would not operate. After an investigation it was found that someone had hung a bundle of SO cable off the pull cord on the slope bottom. This activated an alarm in the hoist house. This required that numerous features of the hoist had to be reset.

WE WERE ATTEMPTING TO PULL POWERED COAL HAULER OUT OF THE MINE. WHILE PULLING CAR UP THE SLOPE THE HOIST THERMALED OUT. THE HOIST WAS ALLOWED TO COOL AND THEN OPERATED PROPERLY.

It appears from acc.site that ee was moving continuous miner from face of #3 entry outby about 1 & 1/2 crosscuts to service it. A coworker said ee had been alone for about 5 min, when coworker found ee pinched between tail of miner & coal rib about 15 ft outby intersection for crosscut #29, entry #3.

EMPLOYEE WAS USING HIS FOOT TO GUIDE THE BELT SHEAVE ROPE ONTO A BOTTOM ROLLER WHEN HIS FOOT BECAME ENTANGLED IN THE ROPE. THIS CAUSED A FRACTURE OF THE LOWER LEG. THIS RESULTED IN LOST WORK DAYS.

As he raised the bolter boom to couple everything together, this caused the drill steel to bow and when it slipped off the lip the steel flexed striking him in the right hand palm as he was reaching for the drill steel. He was not able to work his next shift because of fractured bone next to his thumb.

Employee was preparing to start an end dump truck that had been repaired. As he walked in front of the truck he slipped on the ice/snow and fell on his back. (diagnosis - herniated disc) This is a LTA.

Employee was struck on the right elbow by a rock falling from the mine roof. This resulted in a fracture of the right forearm and lost workdays.

Employee was loading the roof bolter when he stepped on a rock causing him to twist his back. He has been diagnosed with a lumbar strain that has resulted in lost days.

He proceeded through the intersection, battery end first, turning left. When his trailer tire came to the corner he noticed it was going to rub the rib but continued to slide the tire around the corner which popped some of the rock off and shot it towards his operator's cab striking him in the forehead. Cut above his eyes required some stitches. Use protect safety glasses.

EE exited the left side of cab. When reentering cab, EE placed left hand on door jam while slamming the door. He caught his left thumb between the door and the jam. He had 3 broken bones which required surgery to repair.

Employee was moving the miner from #7 to #6 face through the last open crosscut. A rock fell out from between the roof bolts striking his head and sliding down his back. The rock was approximately 3"" thick x 2' wide x 4' long.

Two employees were bolting at the face. When finised in one area, operator lowered the canopy and was suppling his tray with roof bolts for the next place. As he laid some roof bolt on the tray he bumped his elbow the canopy. He had small 1/4"" cut. On Saturday it became swollen and he was ill with fever and nausea. He was treated at ER, released; no missing time until 12/13/04.

EE walked up to the miner in #3 entry, a roof rock fell out from between two rows of pins striking EE on the back, hip and leg. Ee was transported to hospital by stretcher and ambulance.

Returned on 11/18/04, worked a full shift but the leg was swollen. Saw company physician on 11/19/04, diagnosis: cellulitis infection. Issued antibiotic and ordered to stay off of it. He returned on 11/23/04.

THE IMPACT CAUSED A WHIPLASH EFFECT; THE MUSCLES IN EE'S NECK STARTED CRAMPING AFTER THE SUDDEN IMPACT TO THE SIDE & BACK OF THE COAL HAULER CAB. SCOOP DRIVER DID NOT SEE THE COAL HAULER UNTIL THE IMPACT.

He was transported to St. Mary's hospital, where diagnosis to have dislocated his right elbow, and fractured the 2,3,4 and 5 Metacarpus bones in the left hand. The elbow was reset and surgery was performed on the hand on Jan. 30th.

An appointment with an orthopaedic surgeon was on 2/16/04. He was released to return to work with restrictions of limited standing for 2 weeks. He was temporary assigned to an end dump position.

EE was the miner operator and his miner broke down. So he was sent to operate return miner. Other ee told injured to run the miner and he would operate roof bolter. Other ee took off remote control box, noticed he still had communication radio. He tossed radio to injured and struck him in upper lip resulting in a laceration that required 4 stitches.

He cut out the flange on the tooth, took a 10lb. sledge hammer and struck the top back edge of the tooth to knock it off of the shank. As he did a piece mesuring 1.5 cm X .8 cm entered his upper leg. He was taken to Gibson General E.R. were it was surgically removed. It took 6 stitches to close the incision. He was released to return to work on his next scheduled shift.

INJURED WAS STANDING NEXT TO THE SHOE OF THE 2570 DRAGLINE AND A EMPTY BARREL ROLLED OFF THE DRAGLINE. AS HE WAS ATTEMPTING TO MOVE THE BARREL, HE SLICED HIS LEFT LITTLE FINGER ON THE RIM OF THE BARRELL. HE WAS THEN TAKEN TO THE E.R. AND GIVEN 3 STITCHES.

As he was pulling away from the feeder the cable reel activated taking up the slack trailing cable which caused the cable to come up along side of his cab striking him in the face. EE WAS REMOVING A BELT SCRAPER ON THE MS BELT AT THE TIME OF THIS INCIDENT. HE HAD REMOVED THE BOLTS AND WAS REMOVING THE SCRAPER WHEN IT SHIFTED AND FELL AND STRUCK HIM IN THE CHEST.

EE HAD NUMBNESS AND PAIN IN HIS LOWER FOREARMS, WRIST AND HANDS, WITH THE RIGHT WORSE THAN THE LEFT. HE SAID IT WAS FIRST NOTICED ABOUT ONE MONTH AGO AND GRADUALLY GOT WORSE. HE HAS DIFFICULTY GRIPING THINGS. HE THINKS IT IS CARPAL TUNNEL SYNDROME RELATED TO REPETITIVE MOTION FROM ROOF BOLTING, ETC. HE BECAME A LOST TIME CASE WHEN HE HAD SURGERY ON HIS RIGHT WRIST ON 9/9/04.

Flash burns (welding)

Ee & rest of crew were doing repair work to coal chute(welding). No problems noted until late Sat. evening when he stated his eyes felt like they had sand in them. He was taken to local ER where the eyes were examined & given a prescription, a follow up appt on 4/5/04 with specialist & diagnosed with welding flash & corneal abrasion. RTW next shift.

This is an amended report. Del was shoveling and using a drag to pull material from under the belt. He experienced a sharp pain in his right shoulder and chest while doing this. After medical testing it has been determined that Del's shoulder experienced a degenerative condition of the cartilage and that muscles had been supporting the shoulder before it deteriorated.
