Our examiners found a roof fall during their weekly examination of the 4 West return air courses. The fall was located in #2 entry and extended into the crosscut toward #3 entry at crosscut 116. The fall was located in an entry adjacent to the examiners route of travel. The area was timbered.
At around 4:55 pm the Hart street elevator failed to operate properly. The elevator failed to go up and down in the shaft. The investgation revealed that the brake coil failed and had to be replaced. After the coil was replaced the elevator was returned to service.
One of our examiners found a roof fall during his weekly examination of the 2 Main North seals. The fall was located at crosscut 12 of the 2MN from entry #1 to half way between entry #2. The fall has blocked access to the #1 seal.
An unintentional fall occurred in the 2nd main west entry's at the 24+50' between the #5 and #6 entry's. The fall was 8' high by 18' wide by 25' long. Fall was due to deterioration of roof above bolt anchorage.

Our examiners found a roof fall in the 4 West return during their weekly examination of the area. The fall was located in a crossut between entries 2 and 3 at crosscut 79. The fall was adjacent to the examiners route of travel but did not fall in either entry. The fall has been timbered.

Our examiners found a roof fall during their weekly examination of the 3L4MN (5A)return. The fall was located at crosscut 46 between entries 2 and 1. The fall was located adjacent to the examiners route of travel.
At around 10 :55 am the Hart street elevator failed to operate properly. The elevator cage brake dogs locked and would not release properly. This event occurred during an MSHA quarterly inspection of the elevator. The brake dogs were re-adjusted, greased and the elevator was returned to service.

Upon attempting to start the Main South belt for day shift production, the belt would not start and run. An examination revealed that a roof fall had occurred between crosscuts 48 and 49 on the belt and structure. The examination revealed that a slip was present in the mine roof that caused the fall.

Our examiners found a roof fall on the 4 West at crosscut 150 between entries 4 and 5. The fall was located in a crosscut in the intake. The fall did not extend into an intersection. The fall was in an entry adjacent to the examiners route of travel.
An unintentional roof fall occurred in the Main South Belt Entry at the 122+15 foot mark. The fall occurred due to the fact that the roof strata broke just above the anchor point. The fall area was originally supported with 4,5, and 6 foot conventional type roof bolts. Approaches to the fall area will be reinforced, fall area cleaned up and the fall area supported.
Our examiners found a roof fall in the 3 Left 4 Main North return at crosscut 35, entry 2 to 3. The fall was located in the crosscut and did not fall in an intersection. The fall was adjacent to the examiners route of travel.

An unintentional fall occurred due to adverse roof condtions above bolt anchorage. The fall was at the 23+10' in the #2 entry of the 2nd main west entry's. Fall was 25' long X 18' wide X 8' high.
Roof fall found by mine repairman at 7:30 PM. Fall is on Unit 2, Entry 7 at +1820. The fall is 18 ft. wide, 4-6 ft. high, 50 ft. long. Area was supported with 5 ft. conventional bolts, and 6 ft. double lock bolts. A visible slip ran through the intersection and limestone was absent in the anchorage zone. The fall will not be cleaned up.

An examiner found a roof fall on the 3 West A belt during his roadway examination. He started smelling rubber smoke and traced it to the area where the mine roof had pushed timbers into the wet mine floor. The fall was located at crosscut 25 in entry #4 on the 3 West.

At around 10:45 am the Hart Street elevator malfunctioned. A brake shoe got out of place on the cage causing the malfunction. The elevator failed to stop at the proper level at the bottom landing and had to be raised manually.
An unintentional fall occurred in the 1st N. Submains. The fall was 80' X 18' X 8'. The fall fell in the 24+20' intersection and went outby for 80'.

A pumper found a roof fall while in the 5 West intake. He found a roof fall at crosscut 93 in entry #7. The fall was located in the intersection and was more like a pot out because it was only 4 foot high. It left a number of 6 foot bolts in the roof but pulled a few out of the center of the intersection.
A fall of roof occurred at the intersection, in # 4 entry, # 14 xc of the South East Travelway. The fall was approximately 20 feet in length x 20 feet in width x 10 feet thick. The area was developed in april 2004 and had been supported with 6 foot fully grouted roof bolts.
The Main East primary escapeway in Entry 2 between 7 and 8 crosscut was blocked due to roof slough age. Material fell out around roof bolts pulling the plates and heads off of the roof bolts. The affected area is approximately 20ftx16ftx3.5ft. The area will be cleaned up and resupported.

A pumper found a roof fall on the 5 West at crosscut 98 in entry #7. The fall was located in the entry adjacent to the primary escapeway for #2 unit. The area was cribbed and the entry containing the primary escapeway was cribbed as well.
The mine experienced a roof fall in the 5th Northwest Panel at the 8+10 XC between entries #2 & #3. The fall did not affect ventilation, but did involve the secondary escapeway. The fall measured 25' X 10' X 7'thick. The area was developed on 12/21/2007, with 6"" fully grouted bolts. 103(k) issued over the phone.
An examiner found a roof fall on the Main South at crosscut #49 between the roadway and belt line. The roadway had been rehabilitated with 8' bolts and the belt line had been re-bolted with 6' bolts. The fall did not continue into either the belt line or roadway, because of the extra support present.
An unintentional roof fall outby on Unit 1 in Entry 7 between 5070 south and 5135 south. The fall was found by a mine examiner making his weekly inspection. The injured a section foreman was in the process of placing his gloves on the roof bolter when a piece of rock fell from the mine roof and struck him on his left hand. The incident fracured his left pinky finger and cut it. Surgery was required to repair the injury and a portion of his finger was removed due to damage.
The employee stated that he was walking around the metal storage rack behind warehouse when he tripped on a metal band on a skid. He fell striking his left elbow on the ground. At the time of incident power was out on the surface so there was no lighting. This incident became a reportable, when he had surgery on his elbow on 1/16/09.
The employee stated that he was running the continuous miner in entry # 6. He saw that a rock was starting to fall, so he turned to back up to get away from the rock and he felt a pop in his right knee.
Employee stepped on a piece of coal on the mine floor. He twisted his left knee causing a strain. He continued to work the rest of his shift and sought medical attention the next day. He has been diagnosed with a torn ligament in his left knee.
Employee was bolting in #5 entry, putting up wire mesh panel. The panel was crooked and they backed to the last row of bolts for him to straighten the panel and a rock fell out smashing his left hand, breaking 2 bones in his left hand.
Injured man was on his way to work for the 9-19-08 midnight shift at the Francisco underground mine. After turning off a county road onto the mine access road he failed to yield to a large coal hauler truck coming from the left. The large truck struck his vehicle causing several injuries.
Employee was installing a bolt into bolt hole and a burr or something else caught his glove pulling it off his hand. He got a laceration to his left ring finger that required 4 stitches to close, and also fractured same finger where finger joins palm of his hand.
Employee backed over a large rock while operating a D11 dozer causing him to be jarred in the cab. He was wearing a safety belt at the time. The following day he had some discomfort in his neck and was scheduled for an appt. An MRI evaluation later indicated a cervical disc displacement and he was ordered off work effective 10/24/08.
Injured was roof bolting in entry #6, on unit #3, inby crosscut #21 when this incident occurred. He was bolting when the unbolted cut started to work and rocks started to fall. As he moved away from the bolting machine from under the bolter canopy a rock fell in front of him and landed on its end on top of his right foot, fracturing three metatarsal bones in the process.
Mechanic was attempting to put in bearing spacers. He hoisted up cylinder with his crane, the cylinder slipped catching his hand & striking his elbow against the tire. The incident was reported to management on 07/21 as a first aid injury. On a followup visit to the orthopedic specialist an x-ray was taken (8/12/08) & a fracture of the rt. forearm was indicated.
A roof bolter operator was installing his second roof bolt in a row, when a piece of a coal rider fell from the mine roof and struck the canopy of his bolter. As he turned to run away from the machine, a piece of the rock broke as it hit the canopy and struck him in the lower back. The incident resulted in contusions to his lower back.
The injured was operating a scoop and was in the process of pulling into a crosscut to get some boards to build cribs with. As he drove the scoop into the crosscut, he ran over some loose roof bolts and one of the bolts came up into the operators compartment and struck him in the mouth. The incident cut his lip and broke off four teeth.
Injured employee was standing on the back of a golf cart flagging some bad top conditions and fell off the cart to the mine floor. He received a fracture to his right wrist.
A roof bolter operator was in the process of prying down a loose rib at the time of the incident. When he pryed the loose rib down, a piece of rock above the coal fell from the rib, striking him on the right leg and knocked him to the mine floor. Fractured his leg below the knee.
EE was in the process of repairing or replacing a grease hose to a pillow block bearing. The bearing was on a hold down roller on the overland ""C"" belt. The belt was running at the time. His left hand and wrist came in contact with the belt and roller, causing a laceration to his left wrist and a tear to the palm of his left hand. He was sent to local medical facility.
EE was kneeling on top of the #19CH. He then stepped down to the mine floor when his foot came down on a rock about the size of a baseball. His ankle bent one way and his knee went the other.
Operator was spotting a bolt. Trying to get a wrench connected up with the steel and drill pot. He thinks he hit the rotation when he had everything connected.
The injured was brushing float dust from the outside rail of the belt structure, the wire brush he was using came in contact with the outside barrel of a top belt roller, causing the wooden handle to strike his left index finger. The injured received three sutures to close the wound.
The employee was working on a roof bolter, removed a hose from a drill feed jack and the drill feed boom fell on his left arm and hand.
The employee was cleaning in the area of the South East Submain discharge. As he was leaving the area, he twisted his ankle. The employee felt a pain in his foot but completed his shift without reporting the injury to management. When he was examined it was determined that he had a fracture in his right foot.
The injured was standing at the operators controls of the roof bolter when a piece of roof rock measuring 53""L X 34""W X 14"" thick, fell from between the rib and first pin. The rock caused the injured to fall to the mine floor with the rock across his legs. The injured was diagnosed with a fracture to the right fibular head.
Employee state he had bent down to stand a 60"" roller against rib. When he started to lift, he felt something pop in his right arm, between his elbow and bicep. This employee didnt start missing work until he went in for surgery on 10/6/08.
The injured was escourting MSHA inspector, he slipped in some mud, as he fell he caught himself with his right hand injuring his right wrist. The incident was reported but no medical treatment was sought. On 4/22/2008 the injured had x-rays that revealed a partially healed fracture. No work missed until surgery performed on 5/15/2008.
The accident occurred in the slope approx. 1260' from the surface when the examiner was heading down the slope in a Wallace 6 man diesel mantrip. The injured lost control of the vehicle, and struck on of the supporting arches with the front drivers side of the mantrip. The injured walked to the surface and called for help.
While attempting to align a dragline hoist motor to a motor base (<1 inch) using an overhead crane, the motor moved 42 inches horizontally until it made contact with an air valve with the employee's gloved left hand caught in between. The impact resulted in a fracture to the left index finger.
While cleaning ice from frame of belt line at bin roller, employee's hammer became entangled between belt and roller pulling hammer and right hand of employee in behind it. Employee sustain an avulsion to the right middle finger that required surgery to repair.
The injured was lowering the ATRS on roof bolter when a piece of roof rock measuring 36"" X 32"" X 0 to 4"" fell from a slip striking the injured on his hard hat. The injured complained of neck pain and was taken to Provena Emergency Dept., and later diagnosed with a neck strain.
The injured had finished installing his two roof bolts and was watching his buddy install his bolts. A rock fell from in front of the bolter ATRS flipped back and struck him on the left calf muscle area of his leg. The action bruised his calf muscle. He went to a follow up appointment on 12/5/2008 and was removed from work.
Miner operator was hit on shin and ankle of left leg by rock. Causing a sprained ankle and contusions to shin on left leg.
On 7/16/08 the injured was a passenger in a 14 man diesel mantrip. The mantrip he was traveling in attempted to go around a parked scoop when the passenger side bumper came in contact with the scoop. The injured bumped his left shin on the metal partition between seats. On 7/31/08 the injured was diagnosed with cellulitis to the injured area.
MRI of his lumbar spine. blging small disc l5,s1,l4-5. some degenerative changes in his lumbar spine.....EE was assisting 2 mechanics install a hydraulic pump. He was in a sitting position beside the pump as it was lowered into position. Other was moving the pump back and forth to lines the shift spines so it would slide into position. Stiffness and pain in lower back
Employee was returning tools to his truck after repairing a light plant. When he turned to begin walking, he tripped and fell over the tongue of the light plant landing on his left elbow. The area was well lit. X-rays on 3/21/08 were negative. He had an MRI on 4/21/08. He underwent surgery to repair an avulsion of the left triceps tendon on 4/25/08.
Windblown debris struck EE in the back while he was bent over doing general site clean up. THis resulted in a fractured bone in EE's back.
EE originally was injured on 9/6/07. Submission #509993. He had surgery and was released to return to work on 2/5/08. On 2/20/08 he was shoveling drill clippings in a drill hole when he felt pain in the same shoulder. An M.R.I. was performed on 3/4/08, diagnosis was tendonitis, bursitis, and arthritis to the 4 muscles of the rotator cuff group from the previous injury.
On 02/12/08 employee was fueling equipment when he slipped on the ice and fell with his left arm extended which resulted in a shoulder injury. He was taken to the doctor and returned to work with no restrictions. On 02/25/08 an MRI was done which showed a tear of the left rotator cuff. He missed his first day of work on 03/13/08 when surgery was performed to repair the tear.
Changing hydraulic shovel lip and was driving new pins into tooth tips. He was holding pin with channel locks. When he swung the hammer to strike the pin, the hammer head struck the tooth to his right, causing the hammer to strike his left thumb and forced it against another tooth. He was taken to Quest Orthopaedic and diagnosed with a left closed mid-shaft thumb fracture.
Employee was riding out of the slope in a diesel mantrip when the trip started to slid backwards down the slope. After several attempts to stop the trip, the driver turned towards the arches to stop the ride. The impact caused EE, who was sitting in the last seat behind the driver to have contact the back wall of the mantrip.
The injured was making a splice on a miner cable. While stripping the outer jacket, the knife he was using slipped out of the cable striking his leather gloved left hand causing a laceration that required 11 sutures to close.
Employee was struck on top of his left hand by a rock that was approximately 0-1/2 inches thick by 6 inches by 6 inches. He received a laceration to the top of his left index finger and his middle finger. 6 sutures were required to close the lacerations and he returned to full duty.
A plant electrician was in the process of cutting a hole in a piece of belting being used as a splash guard on magnetic separators. His utility knife slipped out of the belting and struck him on the forearm causing a laceration in it requiring stitches to repair.
Employee had just finished installing a roof bolt and walked back to the control levers to drop the canopy, when a rock fell out between roof bolts and struck him on top of his left wrist, causing an approximate 2 inch laceration. The rock that struck him was approximately 0 to 1/2 inches thick and 18 inches x 18 inches. Five sutures were required to close the laceration.
Employee was examining the steering relief valve on a Cat. 793. Oil dipped down onto the brim of his glasses and entered his eye. He cleaned it out and continued to work. On 9/13/08 he reported that the eye was swelling. Taken to ER, diagnosed with infection. Prescribed antibiotic drops and returned to work.
EE & 2 others were taking down Continuous Miner cable from cable hooks. EE's rt glove caught on the latch of a cable hook. As the miner cable fell it pushed the latch of the cable hook into his rt hand. He rec'd a laceration between his rt thumb & index finger. He finished working his shift & sought medical attention after he got home. He rec'd 6 sutures & was released to work
The employee stated that he picked up a 2 wheel assist pump off of a wooden pallet. As he was carrying the pump (approximately 8-10 feet) to a coal hauler, to install he lost his footing and as he stumbled his hand was pinched against the pump and the coal hauler. This caused a fracture and a laceration to his middle right finger.
The injured was trimming a piece of belt to be used for a skirt rubber on the tail piece for Unit #2. He laid the belt across his right leg, and as he was cutting, the utility knife he was using slipped out of the belt causing a laceration / puncture to his right thigh. The injured required three sutures to close the wound.
EE was assisting in the installation of a new foot shaft on a CM.He was using a pry bar to hold the shaft in place while another EE was triyng to drive the shaft in using a hammer.A piece of metal broke off from the hammer or the shaft and struck the injured in the left forearm. (After seeing a surgeon the decision was made to leave the metal in his arm-Misc Mail 7/1/08 #3)
Employee was struck in the left eye by a piece of metal either from a brass hammer or from a roof bolt plate. He was wearing safety glasses but the metal entered in behind his glasses. He continued to work up until 6/18/08 when he sought medical attention to have the metal removed.
Employee struck a pin with a sledge hammer and caught his index finger between the sledge hammer handle and the frame of the truck. The injury required sutures.
Injured had scaled area and replaced a loose bolt and when he went back by bolter tray a piece of rock 55"" X 23"" X 7"" fell from the roof, striking him in the lower back and he fell, hitting his nose and upper lip on bolter tray. 5 stitched to upper lip.
A rock fell from an unbolted area and struck a bolter operator. The bolter operator did not receive serious injuries. MSHA issued a K Order for an incident that did not meet the criteria for a serious accident. The operator was examined at a local hospital and returned to work on his next scheduled shift. We are contesting the issuance of the order.
Employee was loading a roof bolter on Unit 1. He was reaching over onto a scoop bucket to pick up a bundle of roof bolts when his left forearm came in contact with a metal band used to hold the bundle of roof bolts together. He received a laceration to the top of his left forearm just above his wrist. 9 sutures to close the wound.
Employee was roof bolting on unit 1. As he was drilling a hole a fitting broke off a trunion jack and sprayed out oil. As he tried to get out of the way he stumbled and struck his mouth on the drill pot. He suffered injuries to three fron teeth. He had to have one tooth extracted.
EE was roof bolting at the time of this incident. He stated that he was in the process of drilling a hole for a 6 foot bolt, when a small rock fell from the roof and struck his drill pot. The rock landed on the spinnig plate that holds the chuck. This caused the rock to fly off the drill pot and strike him in the mouth, breaking a portion of his left front tooth.
While the employee was taking the bucket off of the diesel scoop, the quick attach pin on the left side stuck. After removing the bucket they tried to move the pin back the opposite direction. They used a brass sledge and a B&O hammer to free up the pin. After hitting the B&O hammer, a splinter of steel broke off of the hammer and struck the employee in the back of his hand.
Employee was roof bolting in Unit 2 on the right side in Rm 2. A rock fell out between the rib pin and center pin and struck him on the right side of his head and right shoulder. He received an abrasion to the top of his right shoulder and a laceration to the right side of his head requiring 4 sutures. He was released and returned to work the same shift.
The injured was roof bolting in Unit #2. He left his starter steel in mine roof unattended while checking suction in his pusher steel. The starter steel fell from the mine roof pinching his middle finger of right hand between the two pieces of drill steels. Required 5 sutures to close injury. The injured was wearing metacarpal gloves.
A roof bolter operator was in the process of adding an extension to his drill steels to finish a drill hole for a bolt. During this process, the drill steels fell from the mine roof, striking the injured on the left middle finger causing a laceration on it. The individual was wearing metacarpal gloves at the time of this incident.
Employee is a Supervisor who had years of repeated overhead curtain hanging & sight markings. On 11/10/08 he felt pain in his left shoulder. He was diagnosed with a lateral impingement and moderate AC joint arthritis. He worked up until 3/27/09 and was taken off work for surgery on his left shoulder. He had left shoulder arthroscopy on 3/31/09.
EE has had 3 sessions of cortisone shots from October of 2007 til August of 2008 for Lateral Epicondylitis (Tennis Elbow). He went in for surgery on 10/13/08 for his left elbow. This illness is caused by reported twisting movements of hand, wrist or forearm done during everyday activity.
EE had pain in his right knee roof bolting.Investigation showed he had surgery on knee in the past and his knee has deteriorated to the point where he is a candidate for knee replacement.Condition is a result of the repetitive motions of roof bolting and the wear and tear on a previously injured knee.
Employee medical examination indicates a sufficient degree of pneumoconiosis. He has exercised his option to work in a low dust of the mine.
This person normally roof bolts and on this date, he felt a burning sensation in his right elbow as he was installing a roof bolt. He was later diagnosed with tennis elbow which is a repetitive motion type illness. He had surgery on 6/26/2008 to repair the problem.
In the process of setting a hietzman jack in an area of unsafe roof the injured was struck on the left elbow by a piece of roof rock measuring 10"" X 8"" X 2""; the injured suffered a chipped bone near his elbow. The incident occurred on the Main North, Entry #3, Sta. 6+40.
