Accident type, without injuries

A fall of roof occurred in the 3rd Right panel in entry # 3, crosscut 49. The fall measured approximately 70 feet in length x 20 feet in width and 6 ' thick. The area was supported with 5 foot fully grouted roof bolts.

A fall of roof occurred in the 3rd Right Panel off the Northeast Mains at crosscut # 43 Entry # 8. The fall measured approximately 40 feet in length x 20 feet wide x 8-10 feet thick. The area was supported with 5 foot fully grouted roof bolts.

A fall of roof occurred on the Main South Beltline at crosscut # 12-13, at 9+50 station. The fall measured approximately 100' long x 18 feet wide x 12-15 feet in thickness. The area was supported with 6 foot fully grouted roof bolts.

A fall of roof occurred in the 2nd Right Panel off the 3rd Southwest Submain at cross cut #67, #2 entry. The fall measured approximately 60 feet long x 19 feet wide x 8 feet in thickness. The area was supported with 5 foot fully grouted roof bolts.

An unintentional roof fall occurred on Unit #1 in Entry 5 @ 37+30. The fall occurred due to the strata broke in the center of the intersection above the anchor point. Several roof bolts were left hanging in the roof. The fall is approximately 0 - 5.5 ft. high, 18 ft. wide, 18 ft. - 20 ft. long. The fall will not be cleaned up.

An unintentional roof fall occurred on Unit #1 in Entry 8 @ 38+50.The fall was found by a mine examiner. The fall occurred due to roof slips coming together in the intersection. The fall is approximately 30 ft. long, 18 ft. wide, 6-7 ft. high. The fall will not be cleaned up.

Accident type, without injuries

At 10:17 am on 5/31/2012 the mine encountered a failure to operations to our elevator. After troubleshooting the elevator, it was found to have a blown control fuse. The fuse was replaced and the elevator was put back in service at 1:00 pm on 5/31/2012.

An unintentional fall occurred at the #82 x/cut, on the 2nd west travelway. The fall was 20' X 20' X 10'.

A fall of roof occurred in the 2nd Right panel off the 3rd Southwest Submain at crosscut # 47, # 6 entry. The fall measured 25 feet long x 30 feet wide x 12 feet thick. The area was supported with 5 foot fully grouted roof bolts.

An unintentional fall occurred at #20 crosscut on the Main North entries @ 14+50' between #5 & #6 entry. the fall was Approximately, 18' wide by 20' long by 10' high.

An unintentional roof fall occurred on Unit 3 in Entry #4 at 66+00. The fall is approximately 8-9 ft. high, 17-18 ft. wide, 50 ft. long. The fall will not be cleaned up. The fall area has a slick sided slip running through the intersection.

A fall of roof occurred in the Southwest Sub main at crosscut # 51, entry # 5. The measured approximately 25 long x 25 feet thick x 7 feet thick. The area was supported with 6 foot fully grouted roof bolts.

A fall of roof occurred in the 1st Right off the 3rd Southwest submain at cross cut 41, # 7 entry. The fall measured 18 feet wide x 18 feet long x 10 -12 feet thick. The area was supported with 5 foot fully grouted roof bolts.

An unintentional roof fall occurred on Unit #3 in Entry #3 at the 59+40 survey station. The fall occurred due to the strata broke at and above the anchor point. There were two slips on each side of the roof fall. The area had been supported with 6' double lock bolts. The fall is approximately 30 ft. long, 19-20 ft. wide, 0-7 ft. high. The fall will be cleaned up and re-supported.

A roof fall was discovered on the Main South belt line Entry #4. The fall measured approx. 30' X 19' X 4 6' thick. No reportable injuries, ventilation and escapeways not affected.

Hart Street Portal elevator computer experienced a malfunction. The computer was unable to determine the location of the elevator in the shaft. The computer was reset and the problem was corrected. A functional test run was performed and the elevator was returned to service

Struck by falling object

The IE looked up to start drilling the hole for the first roof bolt to be installed in that row, when a piece of rock fell, came between the 6"" x 6"" opening of the hog panel and struck him in the upper lip and front tooth. This resulted in a chipped tooth.

Fall from machine

The injured employee was removing material from the third floor of the Prep plant, a protrusion from a piece of steel he was lowering to the ground caught his jacket and pulled him between the bars out of the hoist entryway, to the ground below. He was transported to Deaconess Hospital with injuries to his right ankle and heel.

Struck by flying object

The IE was preparing to load roof bolts into the supply tray for the roof bolter. When he cut the bands loose, one of the roof bolts came up and struck him in the mouth loosening the bridge in his mouth. It was determined that the IE had set the edge of the scoop bucket down on one of the roof bolts. He had a permanent tooth pulled on 10/4/12, that was loosened during the incident.

Struck by powered moving object

I.E. was checking master switch on #12 Getman tractor when it suddenly rolled forward pushing his left leg under a mantrip that was parked in front of it. The injured sustained a fractured left fibula. The incident occurred on the 3 M.E. at XC #47.

Caught in, under or between a moving and a stationary object

Walking up to mantrip to hook up tow rope. Not knowing the ram car was in reverse coming toward him. He was pinned in between the ram car and mantrip.

Struck by flying object

The injured was operating #61 continuous miner when a piece of rock measuring 3' X 5' X 6"" thick flipped out of the conveyor striking him on his left leg causing a fracture to the tibia and fibula that required surgery to stabilize. The incident occurred on MMU-003

Struck by falling object

Employee was standing along the operator side of his ram car when a rock approximately 3ft. x 3ft. x 2in. fell out from between the rib and rib bolt and struck him on the left side of his head and then fell on top of his left foot. He received a fracture to his left big toe.

EE was helping hold the wrench and belt when the operator was pushing the bolt up in place and then it bent pushing his thumb into the canopy of the bolter.

Injured employee walked up behind a man using a pry bar. The employee with the pry bar turned the bar and when he did he spun it around and hit the injured employee in the mouth. Required 5 sutures to close the wound.

Struck against stationary object

Employee was struck by a roof bolter cable that knocked him into a coal rib. He received abrasions to his left shoulder and a laceration to the left side of his face. He received 5 sutures to close the laceration on his face.

Fall onto or against objects

Employee was on top of a de-energized continuous miner. He stepped up on a flare board on the miner tail and slipped off striking his left leg on the top of the flare board. He received a laceration to his left shin that required 6 sutures to close.

Struck by falling object

Employee was starting to seal a stopping. He was kneeling down and starting to apply plaster, when a rock measuring 20"" X 26"" X 5"" fell about seven feet. The rock struck employee on the head. The roof control and spacing was good.

Struck by concussion

Employee was taking a shower at the end of his shift and felt a slight bulge in his left lower groin. He did not feel any pain and had no specific event that caused the bulge. On 4/23/2012 he notified his supervisor, but could not recall any event. He went to the Doctor and was diagnosed with an left inguinal hernia. He continued to work until he had surgery on July 5, 2012.

Fall to the walkway or working surface

The injured was walking next to the #38 Roof Bolter preparing to spot bolts on a corner. A piece of roof rock measuring 48"" X 19"" X 9"" thick fell from the mine roof causing the injured to fall to the ground. He suffered a compound fracture to his left middle finger. The incident occurred on MMU 001-0, XC #152, between #7 and #8 entries.

Caught in, under or between a moving and a stationary object

An employee was removing pull ropes from the miner cable. During this time the cathead was being pulled to the new power center location by another employee. They pulled cable approximately two crosscuts when they heard the injured employee yelled ""stop"" on the radio. They went to the injured employees and found the miner cable wrapped around injured employees right leg.

Struck against stationary object

Employee was in the process of cleaning entry 0 with scoop. As he was scooping inby he tore down part of the curtain. Employee backed scoop outby the curtain he tore down. Employee climbed out of the scoop to go rehang the curtain. As he walked over to the curtain to pick it up employee stepped on a lump of coal and twisted his right knee. Employee had surgery on 4/6/12.

Struck by falling object

The injured employee was standing on the outby corner of the #4 entry at XC-58 on the MMU 001-0 working section. While waiting on the continuous mining machine to be repaired, the injured was struck by a falling rock. The rock fell from the mine roof which measured approximately 8.5 feet high. The rock measured ~ 4' x 2' x 0-4"" thick.

Struck against a moving object

The injured was operating #96 wallace mantrip, the vehicle hit a bump in the road way causing the injured to strike his head on the canopy. The injured was treated for a neck strain and released without restrictions. On 7/13/2012 he was taken off work due to headaches and diagnosed with occipital neuritis.

Caught in, under or between a moving and a stationary object

Employee stated that he was opening a battery lid. The IE had his hand positioned on the other side of the door. When he was raising the lid, the lid shifted smashing his finger between the other lid, causing a fracture to the tip of his left ring finger. The employee had surgery on 2/3/12.

Caught in, under or between a moving and a stationary object

Employee was beginning to start roof bolting. He had inserted a drill steel into the drill pot. He was holding the steel with his right hand when he thought he reached for the lever to raise the pot. Instead he activated the lever to close the auger clamp and it caught his right hand. He received fractures to his right ring and middle finger.

Over-exertion in pulling or pushing objects

Employee was in the process of jacking up a belt tail using a track jack. He felt pain in his right shoulder. He continued to work until 01/20/2012 when he was seen by a physician. An MRI revealed a torn right rotator cuff and a partial tear to his right bicep tendon. Surgery has not yet been scheduled.

Caught in, under or between a moving and a stationary object

IE was backing a tank duster on the travelway using a Getman Tractor, as he attempted to set the duster over the tractor articulated and caught his right elbow between the cab and back section of the tractor. The injured suffered a fracture to his humerus (funny bone) that will require surgery to repair.

Caught in, under or between running or meshing objects

The I.E. bolting glove was caught between his bolting wrench and the drill chuck on the #29 Roof Bolter when he engaged the rotation. His left arm became entangled with the wrench. He sustained a closed fracture to his left ulna and radius.

The injured was struck by a bundle of roof plates that he had requested his partner to toss to him, they struck the knuckle of his left index finger causing a laceration and tendon damage.

Struck against a moving object

The injured employee was traveling up the South Slope operating a Getman Tractor. The tractor suddenly lost all power and the brakes failed to work. The employee and tractor traveled backwards down the slope and struck a coal rib damaging the tractor. The employee suffered a contusion to his left shoulder and missed 4 days of work.

Caught in, under or between a moving and a stationary object

I.E.stated as he was putting a roof bolt in the top he hit his rotation while his hand was on the bolt and it jammed his ring finger on his left hand.

Struck by flying object

The employee stated that as was roof bolting (drilling a hole), the IE was looking up to see how deep the hole needed to be drilled. While he was looking up, the drill pot was approx. at head level or slightly above his head. He was struck by an unknown foreign object that he thinks spun off of the drill pot and hit him in the forehead, causing a laceration that required sutures.

Caught in, under or between a moving and a stationary object

Employee was in the process of putting a cover on the side of a Continuous Miner. The cover slipped and caught his right thumb between the cover and a pry bar. He received a laceration to his right thumb requiring 9 sutures to close.

Employee was hitting with a hammer and a piece of the hammer came off and went into his arm. Surgeon had to removed the piece of the hammer. He received 6 sutures.

Struck by falling object

Employee was roof bolting on Unit #3 when a rock fell out and stuck him on the front of his hard hat knocking his hat to the mine floor. As he bent down to retrieve his hard hat a rock 1ft. x 1ft. x 1-2in. fell and struck his on the top left side of his head causing a laceration. He received 5 staples to close the laceration.

EE has been an examiner,he has repeatedly walked miles of U.G.entries.He had previous problems with his right knee including a surgery.The right knee is sch.to be replaced,EE now states he has pain in his left knee. After exam.by the Dr.determined that the symptoms for his left knee appear from accumulative degeneration.See Dr's report. The EE began missing work on 12/7/12.

EE is a mechanic who has also been a roof bolter has had years of repeated pulling and pushing on pry bars and wenches. On 11/03/12 he felt pain in his right shoulder. On 12/14/12 was diag. with rotator cuff tendonitis, hypertropic degen. changes of the joint, and bursitis. He worked up until 2/16/13 and was taken off work for surgery. See attached MRI results.

Employee came out of the mine complaining of pain in his left shoulder and upper back and neck. He went to the hospital where xrays were negative. The mine has been waiting for doctor's results of an MRI. The MRI revealed degeneration of the Glenoid Labrum with moderate Osteoarthritis. Moderate degenerative changes in C3-4, C4-5, C6-7, C7-T1 and T2-3.

Inhalation of radiations, caustics, toxic and noxious substances

Employee medical examination indicates a sufficient degree of pneumoconiosis. He has exercised his option to work in a low-dust area of the mine.

Employee was struck on the right arm & shoulder by a piece of roof rock on May 6, 2012. The attached reports describe that event did not involve his neck or have an impact on his illness. Employee worked up until January 15, 2014 & had surgery on January 16, 2014. The orthopaedic physician diagnosed him with advanced disc degeneration. See attached MRI & post surgery report.

Employee was diagnosed with carpal tunnel syndrome in her right hand and had surgery on 5/18/12.

Employee felt pain in his lower back with no specific incident. He worked until June 22, 2012 when he was taken off work by a doctor. The mine has been waiting on a doctors report which revealed right L5-S1 recurrent disc herniation from a 1995 microdiskectomy and disc degeneration at L5-S1.

Employee alleged Carpal Tunnel syndrome due to working in the coal mine on 4/4/2012. He had Carpal Tunnel surgery on his left hand on 4/15/2013.

Employee was diagnosed with carpal tunnel syndrome in both hands and had first surgery on 11/2/2012.
