LOADING A TRUCK AT THE LOADOUT BINS EMPLOYEE STEPPED ON A HARD DIRT CLOD AND TWISTED HIS LEFT KNEE.

Inhalation of radiations, caustics, toxic and noxious substances

THE EE RAN AN INSTRUMENT IN THE LAB WITHOUT AN EXHAUST FAN. SHE FELT ILL AFTER RUNNING THE INSTRUMENT SO THOUGHT IT WAS THE CAUSE. MSHA STATE & FEDERAL BOTH INVESTIGATED THIS INCIDENT & HAD A NEGATIVE FINDING. AN MSA ORION DELUXE GAS ANALYZER WAS USED TO TEST THE ENVIROMENT. IT SHOWED NO GAS PRESENT
