Over-exertion in lifting objects

Employee was moving a jack hammer at 3:00 a.m. then a bottle of water at 4:00 a.m. After 4:00 a.m., he felt a sore area and sensitivity around his belly button. Employee finished shift and was off for the next two days (incident happened on Monday in the AM, employee returned to work on Thursday at 8:00 a.m. EE's wife brought in doctor's note on Thursday at 11:30 a.m.

Over-exertion (Not Elsewhere Classified)

Not sure of details. I will send an amended report on Tuesday. Employee hurt ankle in Magox bagging. Will return to work on Monday with restrictions.

Employee attempting to release a rock from a head pulley with a bar. The bar hit employees thumb.

Over-exertion in lifting objects

Lifting a cluth into position and trying to get a bolt started, felt something pull in shoulder.

Struck by falling object

Engine hoist/bolt broken; engine fell on right hand.
