Accident type, without injuries

While emptying pulverized coal from storage tanks as part of a kiln shutdown procedure, an excessive amount of coal dust was pumped to the precalcer, resulting in a significant puff. No injuries were sustained as a result of the incident. There was no equipment damage as a result of the incident.

Fall to the walkway or working surface

A production employee was walking through the plant. He injured his shoulder while catching his fall when he tripped over a curb. This incident was initially treated as a first aid but has been elevated to a medical aid when he recently received medical treatment.

Fall to the walkway or working surface

Employee was walking on a 8"" wide concrete curb by 18"" high. He tripped over a piece of rebar that was covered by weeds and fell. He bruised his knee and wrist. Following a visit to the doctor a couple days later, an x-ray revealed a fracture in the wrist. The employee did not lose work hours as a result of the incident.
