Over-exertion in wielding or throwing objects

Operator was using a hammer and felt discomfort in the wrist.

Over-exertion (Not Elsewhere Classified)

A production employee was cleaning buildup from the internal walls of the riser duct when a chunk of coating fell and hit the end of the high pressure water lance. The employee strained the right shoulder as EE maintained control of the water lance.

Fall down stairs

Cement transport driver walking down a flight of stairs after tank wagon was loaded and claims to have slipped on the 3rd step from the bottom of the stairs causing driver to fall. Driver indicated left knee was in pain & outside medical services were contacted. Driver was transported to Trident Hospital. After this, very little information provided to mine operator until 08-20-18.

Fall from machine

Operator was setting up to dump Vac Truck when the vehicle began rolling. The operator tried to get back into truck to stop it when the operator fell off and broke leg. Truck rolled into a berm and stopped.

Fall from ladders

A maintenance mechanic was utilizing a 4-foot ladder to remove siding from a hopper. Employee fell approximately 3 feet and felt pain in lower back. Employee was immediately taken to a nearby urgent care facility to be evaluated and was released on light duty.

Over-exertion in lifting objects

Employee was moving cement bags from one pallet to another pallet when the employee felt a sudden pain in shoulder.

Fall to the walkway or working surface

A maintenance intern was observing mechanics replace pans on a conveyor when a co-worker pinched the intern in jest, startling the intern to the point where the intern's left knee buckled and the intern fell to the ground. The fall aggravated a previous knee injury to the point where medical attention was required.

Caught in, under or between a moving and a stationary object

Mobile Mechanic was working on an excavator removing a cover plate to access hoses when the plate stuck on one side making the Mechanic's hand slip and drop plate then pinching thumb on other side between cab and plate

Bodily reaction, (Not Elsewhere Classified)

Employee was laying down on the floor cutting a belt guard off. Stood up from a kneeling position and half way they felt a sharp cramp like pain in the lower back. Employee notified Supervisor at lunch time and decided to sleep on the injury to make sure if it went away overnight. Employee then notified the supervisor the next day that it had got worse and called Health Bridge.

Mechanic trying to remove coupling hub from head shaft of limestone feeder. Using a strong back with a port-a-power ram and 1/2"" all-thread rods to rig coupling for removal. While applying pressure to the ram, a rod broke & strong back swung around & hit mechanic on left hand. Treated as first aid but did not improve, instructed to go to doctor & x-ray found broken bone.

Absorption of radiations, caustics, toxic and noxious substances

Worker has been working in Airslides and the cement dust irritated arms and created a rash. EE was prescribe a medication to help with irritation.

Employee was using the bathroom preparing to go home at the end of shift. When ee felt a bite or sting on neck from an insect. The employee clocked out and start to drive home when ee had an allergic reaction causing face and arms to swell. So ee went to the emergency room for treatment.

Miner was entering an elevator when the door shut unexpectedly hitting their head and causing discomfort on neck.

Contact with heat

Employee was working in Finish Mill building when EE started feeling dizzy. EE walked outside to get some fresh air. EE sat down to rest and tried to stand up EE realized EE was too dizzy, nauseated, and could not see very well. EE then laid down on the walk way until help could arrive and went to ER. Note: Employee did state at beginning of shift that EE was not feeling well
