Struck by falling object

EE was struck by a piece of roof rock that fell on roof bolter and slid down his knee. Injury resulted in miner treated and required to ambulate on crutches. This report is being generated under protest to terminate citation #9006089.

EE was driving #3 shuttle car back to the feeder. The back wheel guard came off striking him in the head, neck and right shoulder. This report is being generated under protest to terminate citation #9006020.

Working the ratchet on a chain hoist when ratchet broke and the chain wrapped around his arm NOTE: EE worked did not file the inital incident report with the company until 8/12/2010. EE continued to work until November 16, 2010 at which time he went to see his doctor who restricted him from working.

Struck by falling object

Employee (Miner Operator) was cutting in #3 Entry on the 003 section. The continuous miner had dislodged a roof bolts while traming to the face. The result was that a piece of rock secured by the bolts fell and hit him.

Struck against a moving object

While Traming a scoop, the scoop hit a hole in the roadway and threw the scoop operator into the canopy of the scoop.

Over-exertion (Not Elsewhere Classified)

Injured was with crew loading belt structure into scoop bucket. When EE twisted his body his foot remained stuck in approx. 8" of mud, twisting his left knee.

Fall to lower level, (Not Elsewhere Classified)

EE was plastering a stopping while standing on a bucket. Fell off bucket twisting his knee. Injury resulted in miner treated and required to ambulate on crutches. This report is being generated under protest to terminate citation #9006088.

Struck against stationary object

EE was pulling feeder cable and hit knuckle on hanging first aid box. Injury resulted in miner treated and ambulating on crutches. This report is being generated under protest to terminate citation #9006088.

Struck by falling object

Rock fell from the roof while swinging the drill arm on the bolter. The rock struck the miner on the right elbow resulting in a laceration.

EE was pulling a piece of loose roof rock, when the piece fell, pushed slate bar into right knee. Injury resulted in the application of a knee immobilizer brace and crutches. This report is being generated under protest to terminate citation #9006084.

OUTSIDE OF OFFICE STATED A DEER SPOOKED HIM AND HE SLIPPED AND HURT ANKLE AND POSS NECK.

EMPLOYEE WAS WORKING OUTSIDE AROUND THE MINE AREA WHEN A DRILL POT HIT HIS HAND AND INJURED HIS LEFT THUMB. HE WENT TO APPALACHIAN HOSPITAL FOR TREATMENT, WAS SEEN AND RELEASED.

Absorption of radiations, caustics, toxic and noxious substances

EE was building a ventilation stopping, piece of sealant blew in his right eye. Injury resulted in application of prescription medicine. This report is being generated under protest to terminate citation #9006082.

Struck by falling object

EE was putting up roof bolt, a rock 2' X 3" X 3" fell on arm and leg. Injuries resulted in application of splint and sling. This report is being generated under protest to terminate citation #9006081.
