Struck by falling object

Victim was cleaning rocks and coal to set timbers along a rib line. The rib and brow fell crushing the miner. The brow measured approximately 100" long by 32" thick, by 37" tall.

Over-exertion in lifting objects

While lifting miner cable experienced back pain that extended down his legs. Employee continued working until 01-03-2012.

Lifting Belt Rails. The rails were stuck together and he strained his abdomen

Walking by belt feeder, tripped and fell on left knee - twisting knee

Struck against a moving object

While pulling rock off of the Continuous Miner with the scoop, the rock broke and landed in the scoop bucket bouncing the scoop operator (victim) against the roof.

Victim was cutting a clip off the belt when he slipped and the knife cut his hand.

Shoveling belt - Twisted back while throwing material onto the belt

Struck by falling object

Employees were re-setting timbers in the intake. While attempting to set a 14' timber, they lost control and the timber fell over and struck the victim on the back and pushed him to the ground.

Miner was installing the last row of bolts in #6 rt. While pulling a piece of rock in the face, the rib brow on the left side of the entry fell, hitting him on the rt. ankle.

Victim was removing rock from the continuous miner when a piece of rock fell between the bolts and struck him in the left shoulder and neck.

While installing roof bolts, he stepped forward to insert glue in the hole and twisted his knee.

Caught in, under or between a moving and a stationary object

Holding a board over a kettle bottom for the other bolt operator to bolt up. When the stab jack on the machine was set, it pushed the board against the roof pinching the victims finger

Struck against stationary object

EE stated: After getting off mantrip and walking approximately 15 feet, stepped on rock injuring his ankle. This report is being generated under protest to terminate citation #9006092.

Struck by rolling or sliding object

Removing rock from miner with slate bar when rock measuring 9"x 6'x 8" slid off the miner and struck the victim on the hand.

Caught in, under or between a moving and a stationary object

While cutting a one ton bag of rock dust into the scoop bucket the ram pushed back and pinched the left index finger between the ram and the scoop bucket as the rock dust entered the scoop bucket.

Cut outside of thumb while making a cable splice.

Victim hit his left hand with a hammer. His safety glasses were fogged up

Over-exertion in wielding or throwing objects

Miner was making a splice on a continuous miner cable. While stricking the anvel, a piece of the hammer chipped off and went into his wrist.

Struck against a moving object

EE turned corner in shuttle car hitting rib, straining his neck. Injury resulted in application of cervical collar. This report is being generated under protest to terminate citation #9006096.
