Over-exertion in lifting objects

While lifting a 5 gallon can from the bed of a truck the employee strained his lower back.

Over-exertion in lifting objects

While putting cement bags on packer he twisted lower back. Rx - Naproxen, Tylenol, Acetaminophen, Banalog, Soma, Vicoden, - Receiving therapy.

While lifting a full bag of cement from the pallet, he felt a strain in his lower back. Was given off work order from Doctor - 9/13/05. Rx - Naproxen, Soma, 1/H Pack, Banalg, Lumbar Support, Acetaminophen, Vicodin, cane.

Employee was grinding a belt clip on the Rock Stacker when a part of the splice clip struck him in the chest. This caused a contusion to the right side of his chest. This was not reported until 12/06/05.

Caught in, under or between a moving and a stationary object

Employee was removing hard material from #10 Spitzer belt pulley when he caught his right little finger between the hammer and pulley. This resulted in a contusion. This was originally reported as a (report only).

Contact with hot objects or substances

Changing a Gudgeon bolt in # 4 dust chamber when there was a dust purge and it burned his neck and left ear.

Changing a Gudgeon bolt in # 4 dust chamber when there was a dust purge and it burned the back of my left wrist.

Employee slipped while using a bar to knock down loose material and fell against a ladder causing a bruise to his back and hip area.

While working in the skip basement, employee got a piece of hot clinker on the top of left foot, causing a burn.

Employee slipped on some loose clinker in the storage area and strained his lower back.

A blocking timber fell on his foot.

(REPORTED AS A RECORD ONLY INJURY 8/25/05, REQUEST FOR MEDICAL ATTENTION 8/26/05) Employee was securing vacuum hose for labores when it flushed and struck him on the outside of right knee. This caused a strain.

Employee was checking the operation of the stacker and when he bent over he felt a pain in his back.

While attempting to open cement cooler door, with a sledge hammer, employee caught his R/ring finger between the door and sledge hammer.

Struck by falling object

While working in #3 cooler installing striker plates overhead, something fell into my left eye. I had this removed at the Dr.'s office.

Absorption of radiations, caustics, toxic and noxious substances

The employee was cleaning out the cooler on #4 Kiln and the following morning experienced burns (rash) on his neck. This was no doubt caused by hot dust.

Employee was getting out of the Bobcat when she felt a pain in her left knee. This was later found to be a strain.

Employee struck the feeder slide gate with a hammer causing hot dust to contact his face below the eyes, this caused some minor dust irratation.

Injured was cleaning up around #5 using a wheelbarrow and sprained his upper back.

While working in #4 seperator, employee strained his right hip when lifting out the fan blade.

Employee claims that while walking he stepped on some cement chunks and bruised the sole of his right foot

Using ladder at Gyp bin when Left foot got stuck causing a slip that strained Right big toe.

The employee is not sure how he injured himself or what time during the shift that it happened.

Employee was climbing into a dust truck when he slipped and bumped his Left knee. He reported this as a report only. He requested medical attention on 3/30/05

Employee claims that when he got up in the morning his back was sore. He had been working on the screens in a wet dust collector (Finish Grind) and was not sure if this caused the injury. He was treated for a lower back strain.

Employee fell while swinging a sledge hammer at Gyp bin, straining his back.

The employee stopped his truck in order for another person to get out. When the employee got out he shut the door and that activated the drivers side air bag. The air bag then made contact with his right arm burning and lacerating it.

Contact with electrical current

While working on electrical cart in shop rewiring tail lights, a wire started to smoke. He reached to grab the wire to remove it from the power source and it burnt through his glove and into his finger.
