Employee did not report incident until the following day - 3/20/2013. Employee stated he pushed the door of the Loader open and the door swung back hitting the employee's left elbow.

Employee was using cutting torch to remove slag, piece fell onto a rock and bounced across right eye- mild abrasion to right eye.

Struck against a moving object

Employee was operating a loader, as he pushed the loader bucket into the stockpile, he stopped. During that time a rock rolled from under the bucket to under the left front wheel. As the operator backed away from the stockpile the left front tire drove over the rock, causing the tire to slam down on the ground.

Caught in, under or between a moving and a stationary object

Employee slammed right index finger in door.

5/31/13 at approx. 4:30am, while hammering on the mill liner, a shard of metal came off caused by the concussion of the hammer and impacted the leg of one of the contractors on site. The injury was reportedly minimal and did not require the immediate need of a medical professional.
