Struck against a moving object

Employee backed truck into ballard, causing tightness in his lower to mid-back

Caught in, under or between a moving and a stationary object

Employee was moving a portable welding unit when right hand was placed between trailer tongue and elevator platform-contusion to right hand, index finger

Employee jumped from frame of 992C loader to ground, estimated 3 feet instead of properly dismounting unit. Tail bone came in contact with bolt on frame of bucket.

Employee using table saw to cut wedges and shims when wood bound in safety guard and kicked out causing blade to contact nail portion of thumb.

Struck by flying object

EMPLOYEE WAS WEARING SAFETY GLASSES, NOT GOGGLES AS INSTRUCTED TO CLEAN OUT ELECTRICAL CABINET, DUST GO UNDER GLASSES AND INTO LEFT EYE. INCIDENT OCCURRED, ACCORDING TO EMPLOYEE ON 4/3/12 BUT DID NOT REPORT UNTIL 4/5/2012 WHEN MANAGER QUESTION EMPLOYEE ABOUT HIS Red, irritated LEFT EYE. EMPLOYEE WAS SEEN AND RELEASED FROM DOCTOR ON 4/5/2012 WITH NO WORK RESTRICTIONS.

Struck against stationary object

Employee was bent over, swung around to left and hit his upper lip, left side, on standing guard. Cut required a couple stitches.
