EE was working on a pump. When he stepped on a concrete brace with his inner foot he felt a severe pain in his left knee and noticed unusual swelling. EE was then transferred to a local ER for further tests/treatment. EE was taken off work on 11/30/2010.
EE was descending from the CB Paradise using the ships gangway as he was stepping from the gangway to the cross over bridge, the gangway shifted. EE felt a pop in his left knee while the gangway was moving. EE was taken off work on 11/02/2010 pending surgery.
Two EE's were attempting to clear a plugged sample point piping, which is a routine duty. As they attempted to clear the pluggage, the sample point piping assembly sheared off from the process pipe it was attached to causing caustic solution to spray both EE's. EE's sustained 2nd and 3rd degree chemical burns.
EE slipped and fell resulting in a fractured bone in his right ankle.
EE was performing a routine task consistent with his regular job duties when he felt a pain in his right lower back. EE was taken off work by treating physician beginning 07/05/2010.
As EE was helping the mm's place the hazelton in the hole, he was splashed underneath his left armpit with slurry. EE was treated at medical and returned to work. EE developed 2nd degree burns after second day.
EE experienced pain in his left knee while walking . On 02/02/2011 EE had surgery on his left knee. It has been accepted as a work related injury.
EE was working to clear a pluggage on a bauxite bin when his left middle finger was caught between a water lance and the bin structure. EE was taken to local hospital where Xray was negative for a fracture. On January 17, 2011, Orthopedic Surgeon diagnosed fracture of the left middle finger, making this a Recordable Injury charged to 2010.

EE isolated the air source to an air powered hand tool. EE started to de-pressurize the air hose and tool by depressing the air tool trigger. Air tool socket spun off, striking EE on his right lower leg, causing a deep contusion and a small laceration. On 08/23/2010, treating physician took EE off work due to infection.
EE was installing roof panels and he began to feel discomfort to lower back.
EE states that he was walking, slipped on mud, fell on left shoulder. Did not report until the following day. On Tuesday, May 25, 2010, EE did not report to work as scheduled. He lost work days from May 25, 2010 to May 27, 2010. EE has returned to work with restrictions. Injury was originally classified as non-occupational by the EE.
EE was taking off spare washer, and while taking off the hose and fitting on the caustic shell side washout, he had his hand on the tube directly underneath the fitting and when it came loose, it fell and struck his left middle finger.
EE states he noticed what felt like a blister forming around 14:00 hrs. At 15;30 hrs while changing out of his rubber boots he noticed some blood on his right sock. After removing the sock he noticed a burn and reported it to the front guard gate. EE was seen at medical and transported to the hospital for further treatment.
EE was trying to manually operate a pneumatically actuated valve. When EE applyed pressure to move the manual operating handle, the valve clutch slipped, causing his right middle index finger to get caught between the valve and a bolt. EE sustained a fracture to his right middle index finger. Restricted duty, no Lost Work Days.
EE's left index finger caught between his own body and an adjacent handrail when he lost his balance while pulling on a wrench. X-ray revealed fracture of the tip of the left index finger. EE on restricted duty.
Employee sustained a hairline spiral fracture of his left wrist while removing a clamp. Employee's left wrist was placed in a cast. Employee is currently on restricted duty.
EE sat back on a pallet lift and it toppled forward. It struck EE on the his hard hat and left shoulder region. EE was assessed by Sherwin EMT then was transported to a local hospital for further assessment. EE was placed on resticted duty until release on 09/28/2010.
While washing rail cars EE removed the top seals, as he lifted the doors he felt a sharp pain in the back part of his shoulder. EE was treated at Medical and then was transported to local hospital for further evaluation. EE was diagnosed with acromioclavicular separation. EE has a shoulder/arm immobilizer at this time and is on restricted duty.
EE was in the process of disconnecting a 1-1/2 inch hose from a filter press. The hose fitting fell and caught EE's left hand between the hose fitting and the press structure. Result was a small fracture at the tip of the EE's left middle finger. Finger was splinted and EE returned to work with limitations.
EE was closing an elevator door when the door closed on his right hand. He was place on restricted duty on 08/02/2010.
Initially diagnosed as a right wrist sprain and returned to full duty on 7/19/2010. EE continued to complain about numbness and tingling in right hand. X-ray has revealed an avulsion fracture of the right wrist. EE placed on restricted duty.
EE was performing routine task within his normal job duties when a droplet of caustic landed in his right eye. EE was issued Rx medication.
EE sprained left ankle as he was moving a 55 gallon drum with a dolley. He placed his right foot on wheel axle and positioned left foot back to get leverage. Started to pull on dolley handles and then rolled his left ankle. On 06/02/2010, EE placed on restricted duty.
EE sustained a fractured right index finger while using an 8 lb sledgehammer.
EE stung by a bee on right side of neck below the ear. Released full duty on 4/28/2010
While employee was working to fix platform above 6-1 circulating pump. He was standing on valve handle and circulating pump. He lost his balance while using cutting torch and fell backwards onto his backside. Level of fall was about 2'. On 3/25/2010, EE given minor walking restrictions. Return to full duty on 3/29/2010.
While tearing down a heater, EE's goggles got fogged up. As EE tried to air out his goggles, mud and caustic got into his right eye. EE was treated by an ophthalmologist and issued Rx medication.
EE was traveling down a 3 rung step stool when he skipped last step over-exerting his right leg. He experienced soreness in his right groin area shortly afterwards. Initial diagnosis was strain, no hernia detected during examination. On 1/21/10. He was seen by a urologist who diagnosed him with a mild inguinal hernia. He currently working pending outpatient surgery to repair.
EE was performing routine duties when he sustained a second degree burn on his left lower leg. EE returned to normal duty without restrictions or lost work days on the following day.
EE was using a pneumatic tool to remove scale from the floor of a tank. A piece of scale got into EE's left eye, causing minor irritation. EE was wearing safety glasses with side shields and a full face shield. As a precautionary measure, EE was given Rx medication for the prevention of infection. No lost time or restricted duty.
EE was troubleshooting a portable water pump. In the process of removing the discharge hose from the pump, EE struck his right index finger on the pump casing, resulting in a laceration that required sutures to repair.
EE was using an eight pound sledgehammer to operate a valve when he struck himself in the upper lip with the hammer handle. Laceration required four sutures to repair. No lost time or restricted duty.
EE was conducting housekeeping duties in his normal work area when his left forearm was contacted by escaping steam. EE sustained second degree burns to his forearm.
EE was closing a heater valve when he came in contact with an adjacent heater that had an exposed hot area. The EE sustained a quarter sized second degree burn to his left upper arm. EE was treated at the mine medical center and released to regular duty.
EE slipped on a caustic /scale mixture while working near the sump at 40 area evaporators. This caused him to twist his right knee. Initial diagnosis was a sprain. EE returned to work the same day. On 5/24/10 EE was placed on restrictions pending evaluation of an orthopedist.
