Employee slipped on wet surface and fell to ground. Employee complaining of right shoulder, lower back and right knee pain. Employee placed on restricted duty on 11/26/2013.
Employee was using hydro blasting spin nozzle and the nozzle got diverted and shot employee in left shin with a burst of water lacerating the skin.
Employee was sitting in a chair. He leaned back. The chair tipped back. The employees chair fell back striking the back of his head on the wall where the A/C was plugged in. Employee suffered a 1 1/2 inch cut to back of head.
EE was injured when he fell approx 2' as he stepped off a cross walk. EE sustained a hairline fracture of a vertebra and a shoulder injury. EE starting losing work days on 05/06/13.
EE was exposed to Caustic/Liquor on his face, neck, chest, and into his eyes while attempting to clear a plugged one inch air hose. EE was evaluated and treated by onsite medical staff. EE was then transported to offsite medical facility for additional evaluation and treatment. EE was taken off work on 02/21/13 by offsite medical provider.
Employee was performing inspection of yardrunner, tripped on outrigger. Fell on left knee, fracture of knee.
Operating Long reach excavator in South ""B"" Bauxite Building. Excavator turned over onto its side. ERT was paged out to help EE exit the cab of the excavator. Concusion
EE had energized a 4160VAC contactor and was walking away from it when an arc flash occurred. EE was transported to a local hospital where he was treated and admitted with 2nd and 3rd degree burns to his right arm and 2nd degree burn to his right ear. EE released from the hospital on 10/11/13 but remains off work.
Employee was hosing down under 5-1 heater under the grating. EE was squatting down to hose under the grating, When employee went to get up, he felt a pain on the inside of his right knee as he pushed him self up. Torn ligament right knee.
EE was descending a spiral stairway when he lost his footing and fell. EE's right scapula struck a stairway step resulting in a scapula fracture.
Employee was descending a ladder when he missed the last rung and was suspended by his right arm. EE was taken to onsite medical facility. EE was placed on restricted duty on 06/27/13.
While conducting routine housekeeping duties, EE twisted his back resulting in pain in his lower back. EE placed on restricted duty on 06/17/13 by treating physician.
On 03/28/13, EE reported to onsite medical dept. for groin pain. EE sent out for non-occ bi-lateral hernia. EE could not give a time/date of an injury. On 04/19/13, EE returned to onsite medical facility claiming that the hernia was work related, still could not give date/time. On 05/15/13, EE brought document from physician proclaiming that the hernia was work related.
Employee was descending a ladder and twisted ankle. Employee on restricted duty.
EE suffered a laceration/ fracture to his right foot while struck by scaling bar. The injury occurred when EE accidently struck his foot with a scaling bar. EE was taken to E.R. for treatment. X-ray revealed fracture of metatarsal bones in right foot. Laceration required sutures to repair - ACR
EE complaining of lower back pain. He reported that while operating a front end loader in ship cargo hold #1, the loader struck a protruding object on the wall of the hold. The loader came to a sudden stop and jarred his back. Strained Back.
Employee was backing up a loader in the hold of the ship. The loader bumped into the wall, resulting in neck pain. Employee placed on restricted duty 11/15/2013.
Dry chemical fire extinguishers were used to extinguish the flames. EE was affected by the dry chemicals and smoke and was escorted from the building on the north end for fresh air. Smoke inhalation
While reaching to operate the condensate conductivity block valve, on the north end of 4 digester unit, the EE's shirt snagged on a nearby ball valve handle. When the EE backed away, the ball valve opened, resulting in hot condensate spraying him in the abdomen area. Abdomen burns.
Employees were sailing a ship out of 90 dock when the lines got caught between the ship and the pilings. As they got the line on the dock the ships crew member tightened the instead of giving slack line causing the line to hit and flip another employee off his feet. The Employee that was struck landed on his right side. Contusion right thigh
EE was installing a Level Control Probe J-Box at 45-6 Sump Pump. EE was pushing and adjusting J-Box and got his left ring finger caught between hand rail, unistrut and J-box. On 9/26/13, X-ray revealed a fracture of the left ring finger.
He squatted down to verify the meter was the correct meter, started to move the meter with his left hand/arm and lost his grip, slipping and fell backwards. Employee reached back with his right arm to catch his fall, and as the weight shifted to his right arm, he heard a pop in his right elbow, and immediately felt pain. Elbow strain
Employee was in the process of closing the discharge valve of #1 Spent Liquor Pump by hand when he felt a pop in his lower back. Strain.
While working to rod out feed drain for 2-4 heater to cross block 2-3 heater for isolation, EE stepped onto scale and dirt buildup under flash tanks from cleanup efforts in the area, causing him to twist his right ankle. Ankle Strain
Employee was driving manlift over railroad tracks and felt strain in back when crossing tracks. Back Strain.
While drilling out 3-17 outboard heater drain valve, the drill bit broke and drill and broken bit struck EE in left palm.
Employee was descending the stairs at 50A B Point conveyors stairwell and left foot of his boot caught on a step of the grating causing him to lose his footing. His left foot then slipped off to the next level and heel still on the upper step and as his weight shifted from one foot to the other; he felt a pop in his mid-thigh. Strain.
At or about 1100, EE was rodding out the #12 Washer wash water drain valve, while using a 8lb sledgehammer to knock in the rod, when he felt a pull to his right elbow. Elbow strain
EE complaining about left shoulder pain after conducting a routine maintenance job that required using an 8 pound sledgehammer. EE placed on restricted duty on 08/19/2013 by his treating doctor.
Working at 1-2 Desilicator and was dragging a hose and saw a pipe above and ducked to go under the pipe. EE did not see a second pipe and hit his hard hat on it and then fell to the floor. neck strain
EE was exiting the bed of a pickup truck, stepped in a depressed area of the roadway and twisted her right ankle. EE placed on restricted duty on 7/26/2013.
EE was using a wheel wrench to operate a wheel valve. Wrench slipped and his right hand, fifth finger was caught between the wrench and adjacent flange. EE sustained a laceration that required four sutures to repair. In addition, EE sustained a fracture of the same finger.
While using an 8# sledge hammer to operate valves felt pain in his right elbow. EE stated that the pain got worse when he took a break and rested his arm. EE was returned to normal work without restrictions. On 7/23/13, EE was treated by an offsite physician who placed EE on Restricted Duty on 7/23/13.
EE was closing the suction valve of a pump using an 8 lb sledgehammer. As EE struck valve handle, the valve handle broke, causing the EE to follow thru with his swing. EE experiencing pain in his right shoulder. EE placed on restricted duty on 7/9/2013.
EE twisted his right ankle when he stepped over a pipe. EE placed on restricted duty on 06/04/13.
EE was descending a stairway when he felt a ""pop"" in his right calf. EE was evaluated by onsite medical staff and referred to an offsite medical provider for further evaluation. EE placed on restricted duty starting 4/17/2013.
Probationary employee was in the process of closing an elevator door when his right hand was caught between the elevator doors. X-ray revealed a fracture of EEs right hand middle finger.
EE was struck by a scooter as the scooter was backing up to latch onto a welding machine. EE's supervisor put EE on sedentary duty on 02/04/2013.
EE tripped while exiting crane causing him to slip, landing on his feet, but then rolled backwards falling onto his bottom and lower back.
EE had been using a AEMC Ground Resistance Test meter checking the annual required grounding of electric motors. After three days of this type of work, EE complained of pain in his dominant left hand. EE placed on restricted duty on 01/14/2013.
While walking east on the main roadway between Fac. 45 and Fac. 50 a gust of wind blew particles in EE's right eye.
I picked up a 5 gallon can of oil and proceeded to carry it to the upper level of the 6 A/C/ The area is hot and steamy. After I added oil to 6 A/C, I proceeded to come down. My safety glasses fogged, I missed a step causing me to fall bumping chin on piping. I did not have any other injury other than my chin."" Stri-Strips to chin.
EE was operating a valve that had a defective handle with jagged edges. The jagged edges cause a laceration on EE's hand which required sutures to repair. No lost work days or restricted duty.
Employee was removing metal jacketing from an insulated line. During the removal process, his left forearm struck against the metal jacketing causing a laceration.
Hose busted causing caustic water to shoot in the face of IE.
EE fell backward and landed in a puddle of caustic resulting in a chemical burn of his left Gluteal area. On 04/11/2013, burn area was upgraded to a 2nd degree burn. No lost work days of restricted duty.
EE got caustic in his right eye when he removed his monogoggles. EE was prescribed medication on 03/15/13.
EE was replacing the valve handle on feed pump. As EE was walking away from the pump he struck his head on an overhead support beam causing him to jam his neck. On 01/22/13, EE was prescribed a muscle relaxant and 2 weeks of physical therapy, making this a reportable injury. No lost work days or restricted duty.
Employee has been de-scaling a line with a rivet buster. He set the rivet buster on a pipe in order to take a break. It slipped off of the pipe and landed on the employee's left hand, fracturing his ring finger.
On 05/10/13, Sherwin notified that EE had surgery on 05/07/13 on his left, 4th finger due to an occupational cumulative trauma condition. EE first day missed was 05/08/13.
