Fall onto or against objects

Employee was walking and tripped on grating. Sustained a fractured left patella. Employee required surgery to repair injured body part.

Contact with hot objects or substances

Employee was working on a 300# steam line to remove a skillet blind from the flange when residual condensate sprayed out onto the employees left mid section.

Absorption of radiations, caustics, toxic and noxious substances

Employee was climbing out of sump, when the top of hard hat made contact on top of sump. This resulted in the hard hat contacting the chemical goggles, repositioning them and exposing eyes as the hard hat fell into a puddle of caustic. When the hard hat fell into the caustic, it caused the caustic to splash up and into the employee's left eye.

On 7/12/2016 at approximately 11:15 am, EE (Area Lead) was working the recirculating valve on #3 Deep Bed Washer when the wheel wrench ee was using, slipped off of the valve and smashed right ring finger between the wrench and an adjacent flange. EE immediately went to Medical where ee was treated and released.

Struck by falling object

A wheel wrench fell onto EE's left foot and fractured the fifth toe (tiny toe)

While hosing down the screen box on 10 settler, employee was splashed in the face and eyes with hose water.

Contact with heat

Employee was hosing down inside 10 settler, for approximately 40-45 minutes with 1"" hose down hose, and had PPE on. Employee called to have hose shut off, walked to the man-way. Employee suffered from dehydration from lack of fluid intake.
