A roof fall was discovered in the 5 North #2 area between crosscuts 83 and 84 in the #6 Entry. The fall is approximately 80' long, 16' high and 16' wide. No ventilation or travelways were affected. All accesses have been posted and dangered.
While working in 1 South to correct a stone that had fallen in the walkway, the barometric pressure dropped drastically. The stone in the walkway was decreasing the amount of ventilation. The mine was evacuated due to a methane inundation in 1 South at 11:45 pm.
An area in the 1 South Seal walkway was found to have a stone approximately 10'long, 4'wide and 4'thick. This was not above anchorage but the travelway was blocked temporarily.
A roof fall was discovered at crosscut 9-20 on the mainline haulage. The fall is approximately 16' wide, 30' long and 10' high. Mainline haulage is blocked but ventilation is not affected. The area has been rebolted and is in the process of being cleaned.

The Cameron Portal elevator governor circuit tripped and 2 employees were stuck on the elevator for 32 minutes. The breaker was reset and the employees were brought to the surface.

Employee while stepping back to continue task of cutting fabric protector off a roll of fabric, EE claims to felt pain in knee. Cause of this alleged injury was not paying attention to where EE was stepping. EE did not report this incident until the next day. Compliance with rules/regs., PPE, mining equip./systems not a factor. Miner proficiency may have been a factor.
When stepping off of a motor, the employee stepped on a rock and buckled knee.
While changing a top roller on the #17 belt after it was emptied, locked and tagged out, the come-a-long being used to raise the top belt slipped allowing the top belt to drop. The individual was between the belts changing the roller and the top belt pushed EE's head into the structure causing a laceration to EE's right ear and a fracture to EE's skull.
While walking through crosscut 209 the individual stepped on a roll of belt that had a nail sticking out. The nail punctured EE's right foot.
While uncoupling cars in 154 switch, the pull rope snapped causing the individual to fall into the coal rib jamming shoulder.
EE was delivering stone to the location for Lincoln Contracting. As the bed of the truck was being lowered electricity from the powerline allegedly arced striking the bed of the truck. Upon inspecting for fire at the right rear of the truck, the right front tire exploded striking EE with debris. MSHA jurisdiction is in question by Marshall County Coal Company.
While lifting a base plate onto the longwall shearer, it slipped and struck left middle finger. *Employee did not start missing work until 5/30/2016.
While the individual was traveling as a Federal Inspector escort on the Mainline #7 Belt, EE stepped in a hole and twisted left knee. NOTE: Individual worked 5/11 through 5/13 after the incident. EE began missing work on 5/16/16.
While loading damaged doors from the 15-35 track spur, the individual caught finger between the door and the track car causing a laceration that required sutures.
While traveling to Blake's Ridge, the #1 Brookville motor went into the 15-35 switch. The motor hit the equipment doors causing the doors to fall on the individual creating discomfort in left shoulder and upper back.
While uncoupling hoses and using two hammers, a metal shard broke off and hit the individual in the neck.
While operating a 636 scoop and loading pan line, the piece of pan line on the scoop caught another piece of pan line. This caused the pan line on the scoop to bounce. The scoop car also bounced causing the individuals head to strike the canopy causing neck pain.
While using a jack to jack the track up, the bar slipped and rolled causing a strain to the individual's right shoulder.
While moving a "Z" Drill in the longwall panline, the individual slipped and fell hitting left shoulder and left elbow in the panline.
After loading a rail onto a rail car, the individual was attempting to move the rail to the center of the rail car. While doing so employee pinched employee's right middle finger between the rail and another rail that was already on the car.
Holding a hose while another individual was using a hammer to hit the coupler when a piece of metal sheared from the coupler and went into the injured person's left index finger.
When stepping off of a diesel motor, individual felt a "pop" in employee's left knee.
While uncoupling two man busses that were blocked against motion, the operator of the man bus activated the automatic uncoupler, pinching 4 fingers on the individuals right hand.
Employee was unloading roof support beams from a mine car with another employee. Employee felt a pain in employee's back.
While traveling on #505 Motor on the mainline haulage of 16 track, the motor picked the switch at 16-27 track spur. The motor derailed and struck the inby rib injuring the individuals left wrist. NOTE: Employee did not miss any work until 4/12/16 as a result of this injury.
Employee was installing air and discharge line. Discharge line was completed. The compressed air line had been shut off and bled 2-3 hours before removing the air line end. While removing the end it blew off striking the injured in the mouth.
When loading a beam onto a supply car, employee heard a "pop" in employee's right shoulder. Employee did not begin missing work until 03/08/2016 for the injury.
Employee slipped on ice at the top of the slope while holding handrails. Felt pop in shoulder.

Employees were tossing and stacking 45 pound cement bags and the injured employee caught a bag with EE's right long finger in a position that caused a dislocation.
