Accident type, without injuries

A reportable fire occurred on mine property at the substation of the Cameron Portal. A transformer which is owned by AEP was the source. The fire was discovered at 12:40 and extinguished at 1:18 am. No injuries occurred and the mine was evacuated due to loss of power to a fan.

An unplanned roof fall was discovered at crosscut 17-18 between the track and belt entries. The area is approximately 10' high, 26' long, and 16' wide. The fall did not affect any travel ways or ventilation. The area has been posted and dangered off.

An unplanned fall of an overcast supporting wall was discovered at 6:50 pm at the No. 14 crosscut of the 5 North No. 1 Belt. This was not a fall of the roof as no roof was involved only the supporting wall of the overcast. The wall was rebuilt and the travelway was reestablished.

An unplanned roof fall was discovered in the Conner's Run Return adjacent to the No. 1 Belt Drive. The fall is approximately 12' wide, 20' long and 5' high. Management is in the process of cleaning and supporting the area to restore safe passage.

The No. 77 mantrip was traveling into the mine when the brakes became hot and the caliper caught fire at 15-05 crosscut. The fire was reported to have burned for 10 to 12 minutes before it was successfully extinguished. No injuries occurred as a result of the fire.

An unplanned roof fall was discovered in the Conner's Run return airway at the "Blow Hole". The fall was approximately 11' long, 10' wide and 3' high. It was temporarily blocking travel but has been cleaned and supported to reopen to safe travel.

An unplanned roof fall was discovered at 9:45 am in crosscut 14-35 between the track and belt entries. No ventilation or travel ways were affected. The area is being dangered off from travel.

An unplanned roof fall was discovered at 11:45 am in the Left Side Return airway at No. 49 crosscut. The fall is approximately 30' long, 16' wide and 12' high. The travelway is blocked. A submission has been made to MSHA to make the area common which will permit travel around the fall.

An unplanned roof fall was discovered at 3:25 am in the 17-10 crosscut between the track and belt. The fall is approximately 26' wide 20' long and 12' high. No ventilation or travel ways were affected. The area has been posted and dangered off.

An unplanned roof fall was discovered at 10:45 am at the number 192 crosscut of the 5 North Left Return. The fall is approximately 16' wide 30' long and 12' high. No ventilation or travel ways were affected. All access to the area has been dangered off. The air way is common with another entry so the weekly examination has not been affected.

At 2:45 pm management was notified that one employee had been stuck in the Blake's Ridge elevator for 30 minutes. The elevator car safety switch had tripped causing the incident. The fault was cleared and the employee was brought to the surface at 3:00 pm.

An unplanned roof fall was discovered in the #3 Entry at crosscut 13-09. The fall is approximately 20' wide 20' long and 10' high. All accesses have been posted and dangered off.

Four men were reported as being trapped on the Fish Creek elevator at 12:42 pm. The men were inspecting the elevator when it traveled below the safety switch causing the elevator to be unable to return to the surface. All four men were of the elevator and standing in the Intake until the elevator was returned to service and at that time traveled to the surface.

An unplanned roof fall was discovered at 4:35 pm at 16-17 intersection on the belt line. The fall was approximately 15'high 16' wide and 80' long. The fall was cleaned, bolted and the area was restored to safe passage.

An unplanned roof fall was discovered at 6:35 pm at 16-24 intersection on the belt line. The fall was approximately 12' high 16' wide and 30' long. No injuries occurred. The fall was cleaned, bolted and the area was restored to safe passage.

An unplanned roof fall was discovered at 6:50 am in the 16 East Bleeder. The fall is in the #1 Entry at the #2 crosscut. The fall is approximately 20' high, 15' wide and 80' long. The area has been dangered off and bolting is in process.

A roof fall was discovered at 9:40 pm at crosscut 16-21. The fall is approximately 10' high, 25' long and 16' wide. The fall did not affect any travel ways or ventilation. The fall has been posted on both sides.

Three employees were trapped on the Blake's Ridge elevator at 11:25 am. The elevator was fixed and they returned to the surface safely.

An unplanned roof fall was discovered at 3-25 Mainline above the steel sets. The fall measured approximately 11' high, 16' wide and 15' long. One steel beam was affected. The beam was replaced. No ventilation or travelways were affected.

A fully trained miner injured Rt. pinky finger when loading a ranging arm onto the longwall face a piece of rock fell from between the roof support striking miner's finger. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was walking under the head gate shields when a piece of rock fell from between the shields striking right foot. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner stepped on a bent rib strap causing it to strike EE in the right knee. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was carrying belt structure when hand was caught between the piece of structure and a steel set on the belt line. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was installing a rib pin when after completing the installation EE turned around and struck EE's hand on a drill steel that was laying on the miner. The injury was not the result of any compliance issues nor were any protective guards involved.

Over-exertion in pulling or pushing objects

A fully trained miner injured their back while moving/pulling a ventilation curtain that was covered in loose coal. The injury was not the result of any compliance issues nor were any protective guards involved.

Struck by falling object

A fully trained miner was struck by a rock when installing glue in a roof bolt hole. The accident was not the result of any compliance issues nor were any protective guards involved. The employee received medical treatment as arm was glued for the laceration ee received.

A fully trained miner injured left ankle when ee was attempting to change a hose on an air pump. The employee slipped backwards and felt a pop in ankle. The injury was not the result of any compliance issues nor were any protective guards involved.

The individual was shoveling under the belt, with their back to the rib, when approximately 1' to 3' of rib rolled out. This covered their feet and lower back.

A fully trained miner walked into a longwall shield and strained their neck and right shoulder. The incident was not reported to mine management until 11/27/17 at which time the employee decided to seek medical treatment. The incident was not the result of any compliance issues nor were any protective guards involved.

A fully trained employee has no exact memory of anything left knee began hurting and as the shift progressed the pain increased. The alleged injury was not the result of any compliance issues nor were any protective guards involved. There is no known cause of the injury.

A fully trained miner was installing a roof strap when a rock fell from the roof striking EE in the upper lip resulting in stitches to the lip. The accident was not the result of any compliance issues nor were any protective guards involved.

Caught in, under or between a moving and a stationary object

The individual was setting a steel beam on a structure used to place the beam across the conveyor belt when hand hit the waterline and the beam slipped out. This smashing left thumb. Received 12 sutures.

Struck by falling object

A fully trained miner was injured when miner was putting oil in the continuous miner. While holding the lever for the miner, a rock fell from between the straps striking miner's left shoulder and neck. The accident was not the result of any compliance issues nor were any protective guards involved.

Employee started shift at 7:00am was working on building a Strata Overcast using the Strata Panels. Around 11am one of the panels got stuck and employee kicked the panel to free it up. A piece of wire off the panel went through boot and into the arch of foot. Employee was met at the hospital by Strata Safety Supervisor.

This questionable injury was supposed to have occurred when a fully trained employee was picking up a two gallon spray bottle while cleaning the bathroom. The employee claims that they felt a burn and later when showering they felt a lump in their groin area. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was walking out of the Fishcreek shop when miner stepped on a drain cover and fell striking miners right knee.

Struck by flying object

A fully trained miner was injured when pulling a shield on the longwall face. A piece of dirt got in eye while operating the shield. The accident was not the result of any compliance issues nor were any protective guards involved.

Crew was unloading a steel car and an employee pulled a beam off of the car and it rolled and hit the injured employee's left ankle-foot.

A fully trained miner injured EE's neck and shoulder when a rock fell from the top while EE was bolting a corner in the 5 North Mains. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner injured their back while prying a wheel from a hub. The incident was not the result of any compliance issues nor were any safety guards involved. Employee did not start missing work until 10/02/2017.

A fully trained miner injured Rt. Shoulder when installing supplemental bolts on the 16 belt. While drilling a piece of rock fell from the already supported area and struck Rt. Shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was injured when releasing a breast line on a barge. The barge drifted and caught the employee's hand between the line and the barge. This was not a result of any compliance issues nor were any protective devices involved.

A fully trained miner was injured when EE tripped on a piece of rail causing EE to jam right shoulder and neck. The accident was not a result of any compliance issues nor were any protective guards involved.

A fully trained miner was injured when a rock fell from the top onto the slider tube. The slider tube then pushed the employee against the miner injuring neck and upper back. The injury was not a result of any compliance issues nor were any protective guards involved.

A fully trained miner was building an Omega Block wall when a block fell catching left middle finger between the block and the top of the ladder EE was standing on causing a laceration to the finger. The accident was not a result of any compliance issues nor were any protective guards involved.

A fully trained miner was lacing cable slack on the boom of the loader when employee turned employee's head employee's hard hat hit the cable and felt pain in employee's neck. This was not a result of any compliance issues nor were any protective devices involved.

Employee alleged employee bent over and was pulling a 1 inch hose out of a supply job trailer and received a strain in employee's lower back.

Individual was walking through water approx. 10" deep and slipped and tripped on a submerged airline in the walkway. This causing the individual to fall, hurting lower back and right knee.

A fully trained miner was transporting belt rollers through a mandoor when the roller hit the door frame jamming miner's right wrist. The accident was not a result of any compliance issues nor were any protective guards involved.

Inhalation of radiations, caustics, toxic and noxious substances

Contractor employee was cutting galvanized grating with a torch. Experienced difficult breathing on way home that day. Cause of this respiratory condition was from not wearing proper respirator protection while cutting galvanized material. Did not involve rules/regulations, mining equip/systems but did involve miner proficiency and protective devices.

Absorption of radiations, caustics, toxic and noxious substances

A fully trained miner experienced skin irritation on right arm from what miner alleges was contact with B-Bond while sealing a stopping in the 5 North Mains. The accident was reported to have happened on 01/23. However, no accident report was filed until 01/26. The incident is still under investigation and this report is being filed prior to 10 day requirements to avoid violation.

Employee was coming down a set of steps and tripped over own feet. The employee received a stress fracture right foot.

A fully trained miner was assisting move the fan from 3 to 2 entry when the bologna skin became caught on corner. As EE was freeing the caught piece, a rock fell from between the bolted rib straps and hit left foot. The injury was not a result of any compliance issues nor was any protective devices involved.

Employee was breaking bags of concrete to put into hopper and got chemical burns on body causing injury.

A fully trained miner injured right knee when walking the 5 North #2 Belt. When walking ee slipped and struck knee on the bottom. Employee received Medical Treatment for the injury. The accident was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner strained miners ankle while walking on the belt line. When treated the miner received medical treatment in the form of a splint. The accident was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner experienced blurry vision while running a motor on the haulage. No known mechanism of accident was reported. The employee did not miss any scheduled work but did receive medical treatment in the form of a prescription when treated by a physician. The incident was not the result of any compliance issues nor were any protective guards involved.

Employee was trying to move a garter snake from the work area when they were reportedly bitten. The incident was not the result of any compliance issues nor were any protective guards involved.

while changing top structure, the employee pinched employee's left thumb between the bed rail and the top structure. resulting in (7) stitches.

A fully trained miner was installing a brake unit on a take up when the shaft on the brake slid causing employee's left middle finger to be caught between the shaft and the brake causing a laceration that required sutures. There were no compliance issues nor protective guarding involved and no days were missed.

A fully trained miner was installing a beam when miners right hand was caught on the beam causing a laceration to miners finger that required 8 stitches. The incident was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was struck in the lip when installing a hose on a longwall shield. The hose struck ee in ee's lip resulting in ee getting two stitches. The incident was not the result of any compliance issues nor were any protective guards involved.

Employee was awarded 20% permanent partial disability for Occupational Pneumoconiosis.

Alleged illness was reported by employee while hosing at PP, which the water is supplied by the city, claims legs got wet and over time developed a rash (irritation) that would not go away. Employee sought medical attention and MRSA was diagnosed but ee was not tested to confirm.
