Accident type, without injuries

Water truck driver saw smoke and flames in the area of the primary crusher. When the smoke and dust cleared, an explosion door was found to be off the #2 dust collector

Fall from machine

Elevated position (appr. three feet from ground) Injury jumped from the propel motor on a P&H 495 HR Shovel to retrieve a dropped wrench. He landed with his right heel on the track. His heel was fractured on impact. Swelling and bruising of tendons was associated as well.

Over-exertion (Not Elsewhere Classified)

As a contractor was walking back towards the pump truck, he stepped on uneven ground, causing his right knee to hyper extend. Possible torn middle medulla.

Bodily reaction, (Not Elsewhere Classified)

At the time of the incident the tool room leadman was standing on the lower step of a 4-foot step ladder that was set up parallel to a storage rack system. As the leadman reached back to retrieve a part that was located slightly behind him and to his right, he felt a sharp pain in his lower back.

While setting conveyor tube on supports above railroad tracks LTL crane collapsed causing severe injury to 2 employees of TIC...still under investigation cause unknown at this time

Fall from machine

While lifting a conveyor section for a new coal loading facility the ground beneath the crane subsided causing the crane to topple.

Struck against stationary object

A mechanic was removing a box pin from a haul truck and stepped back onto a small rock, rolling his ankle. The injury resulted in a fracture to the right fibula.

At the time of the incident the technicial was trouble-shooting a power problem with the rear engine of a scraper. After replacing the air filter and fuel filter the tech started to step from the left rear fender to the rear push block. As he was making the step, leading with his left foot, his right foot caught on some dried mud causing his knee to twist.

Fall down stairs

Slipped coming down the stairs at the South Black Thunder Loadout. The building had just been washed down and the stairs were wet. Employee fell down the last 3 stairs, landing on her elbow. Employee was taken to Campbell County Memorial Hospital were it was determined that she had sprained and possibly fractured the elbow. Employee was given a full work release for 3/25/08.

While removing wheelmotor off HT 188 with wheelmotor handler machanic was removing the air hose off top of motor. Mechanic placed right foot in path of drive wheel while the handler was in motion. The handler drive wheel rolled up part way on the right foot causing damage to the foot.

At the time of the incident the service technician was removing fuel lines from the fuel transfer pump of a CAT 793 truck. As the technician was applying pressure to a wrench, the ladder he was standing on tipped causing him to fall to the ground and land on his right wrist.

While loading jeep & booster axles on lowboy trailer-driver thought everyone was aware he was pulling forward-employee was not clear-driver ran over right foot with rear axle tires on left side.

Employee lost her footing on an icy roadway, landing with her right knee under her.

The left ring finger was fractured when it was pinched during a trip cable change on a shovel.

Slipped and fell on muddy slope while approaching haul truck, twisting ankle.

Employee was power washing a vehicle when she stepped backwards and stepped on the edge of the wash pad and sprained her right ankle. The employee was in compliance with company and mine rules and regulations. The accident did not involve the mining system, job skills or protective equipment.

While setting the differential pump into place on a truck, the pump slipped and smashed injury's small digit on his left hand in between the pump, motor, and differential housing. Stitches were placed in the small finger and the wound was dressed.

The becket end of a wire rope struck and lacerated (required sutures) the right outside eye socket while untangling hoist cables.

While installing right front brake assembly on a haul truck, brake assembly shifted pinching the left thumb causing a laceration which required sutures.

Truck driver was chaining transformer on flatbead trailer when the chain boomer slipped, striking the driver above the right eye. Suffered a laceration requiring sutures.

Fall from machine

Employee was dismounting a D11 Dozer. His foot slipped and he fell to the ground. He complained of pain in his right shoulder. Doctor did a CT-Scan and said he had a cracked scapula. He said they heal on their own and that no treatment was necessary.

Struck by flying object

Two employees were welding and cutting on the apron of 6147-4341 Scraper. The welder warned the other that he was going to start grinding. As the employee turned away from the grinder, a piece of foreign material blew in behind his glasses.

At the time of the incident the service technician was removing damaged hydraulic hoses from a haul truck. As the the tech was using a knife to cut the plastic ties that held the hoses in place, the knife slipped and cut his left wrist.

Left hand ring finger became pinched between the drive line yoke and the drive line seal, and lacerated the finger.

While changing a lock ring on a 793 Cat haul truck EE got finger pinched between the lock ring and wheel.

The technicians right index finger was lacerated (required sutures) when attempting to adjust the opening on an air conditioning vent in a water truck.

Struck by falling object

As the service technician was removing a strut pin from it's bore, the pin slipped from his grasp and fell, landing on his right middle finger, pinching it between the pin and a ladder.

Employee was tightening bolts on primary steel; wrench slipped, arm was torn open at bicep muscle and required stitches.

While removing the last bolt on the hitch assembly for the belly dump trailer, the underside of the mechanic's left forearm was pinched. The result was a laceration requiring sutures.

Employee attempted to place a plate under the rail while the rail was being placed, resulting in a rail being placed on the plate with his finger under it. The skin on the ring finger of the right hand was pinched requiring stiches.

Lifting pin to secure a steel choker and the pin came down fracturing the right finger.

An Electrical Planner lost his footing on a steep hillside and pivoted on his right leg to regain his balance. At this time he felt something give in his lower leg and fell to the ground. He finished the shift and went to the doctor the next day, at which time x-rays revealed a fracture to the right fibia.

At the time of incident the technician was replacing a damaged hydraulic hose. As the technician was applying considerable pressure to the wrench, the wrench slipped off the bolt and struck him in the mouth, chipping a tooth.

EE was carrying rebar and slipped on the rebar mat he was walking on. EE fell forward. His out-stretched hands missed the rebar mat, resulting in his face striking the rebar mat. EE sustained a laceration to his bottom lip, required sutures and he also broke a tooth.

At the time of the accident the tech was replacing the air compressor mounting bolts on a CAT 793 truck. To access the compressor the tech was standing on the truck's belly pan. As he was working his foot slipped, causing him to fall and strike his right side against the fuel filter base

Struck against a moving object

Employee was operating the lube truck, hit a bump and hit head on the inside roof of the cab. He was not wearing a seat belt.

EE operating electric drill-bit became stuck-drill was wrenched from EE, causing fracture to back of right hand.

Caught in, under or between a moving and a stationary object

Putting a tow pin into the back of the loader and smashed the ends of two fingers on the right hand between the pin and the eye. As a result of the incident the right ring finger was fractured on the end.

The technician was found slumped over, but conscious, in the seat of his pick-up. The technician was driven to the first aid station where he was transferred to an ambulance. The technician was pronounced dead on arrival at the hospital.

At the time of the incident the technician was installing a hoist cylinder on a CAT 793 haul truck. As the cylinder was being hoisted into place the crane failed, causing the cylinder to fall and strike the technician on his right shoulder.
