Over-exertion (Not Elsewhere Classified)

While walking around the front of the service truck, the technician turned, twisted and slipped, fracturing the lower left leg.

Caught in, under or between (Not Elsewhere Classified)

The plant maintenance technician's hand was pinched between a pulley shaft and a pillow block bearing while installing the pulley. The result was a laceration and two fractured bones in his right hand.

Caught in, under or between a moving and a stationary object

The technician was removing the rear driveline from ST 1921. The mechanic was prying the driveline away from the yoke on the differential as he inadvertently placed his left index and middle finger in the area of the upper u-joint by the carrier bearing. As the bottom end came free, the fingers were pinched causing lacerations, requiring sutures.

The operator was descending a stairway from the cab of a shovel. As he stepped down to the lower walkway, his ankle rolled over. He was first seen by a doctor on 8/20. A follow up x-ray on 8/24 confirmed a fractured fibula.

Struck against a moving object

Employee called supervisor and complained of a sore neck. Supervisor asked what happened and the employee said nothing. No event could be determined by the employee, his neck just got sore. We transported the employee to the hospital where they found no apparent injury and called it a neck strain.

The technician stumbled, fracturing the right fibula just above the ankle as it rolled over when exiting the crew bus.

Caught in, under or between a moving and a stationary object

When pushing up/removing the tub manhole cover, the technicians hand slipped pinching/lacerating his left thumb between the lip on the cover and the lip on the tub.

Struck against stationary object

Employee was removing a retaining pin from a drop chute on a bulk delivery truck when the pin released suddenly causing the employee's hand to strike against a sharp edge resulting in a laceration to the top of the employees left hand. The wound required four sutures to close.

While starting a pin in the apron roller of a scraper, the mechanic struck his right middle finger with a hammer causing a laceration requiring sutures.

Individual was using an impact wrench when the nut broke loose, pinching his finger between the wrench and the transmission. Stitches were required for the laceration.

Struck by flying object

The mechanic was tightening a bolt on a hydraulic steering line when the wrench slipped off striking his left front tooth and chipped it.

Struck against stationary object

While inspecting the air conditioning system unit on RubberTire Dozer #600, the maintenance technician lacerated the middle finger on his left hand on the overhead unit in the cab, requiring sutures.

A wrench slipped off while tightening a hose fitting and the employee lacerated his first and second knuckles on a metal edge. It required stitches to close the wound.

Was scraping mud from stairs, scraper got caught and cut thumb and ring finger.

The technician's palm and knuckle on the left hand were lacerated/pinched when the swing style pin driver he was using glanced off of the wedge in the dump rope socket.

A technician's foot slipped while climbing a short ladder to check the oil on a haultruck. He felt a pull in the bicep of his left arm when he caught himself. On May 7, 2010 he had surgery to repair a bicep tendon.

While in the process of removing a shim from the latchbar on Shovel 26, a technician received a laceration to the inside of his right ring finger, requiring stitches.

Employee was installing propel motor on P&H 2800 shovel. Employee tightened feet bolts with impact wrench. Injured worker was verifying that he had all the bolts tightened by using 2-1/4"" slap wrench and sledge hammer. As employee attempted the bottom foot bolt, the slap wrench came off the nut, striking the employee in the mouth, chipping the top right central incisor.

The #14 dragline oiler worked through the day conducting normal duties as well as assisting to replace the upper spreader bar. The next day he was diagnosed with actinic conjunctivitis of both eyes.

Struck by flying object

Employee changing 3/8"" wire rope cable on Kranco 10 ton overhead crane in Main shop bays 3 and 4. Injured employee and co-worker performing task in the man basket of an S-85 Genie man lift. While setting the hoist lower limit switch the employee caught the tip of their nose on wire rope cable causing a laceration which required sutures

Struck by flying object

While driving a bearing out, a chip broke off and struck right thigh. Individual received stitches to close wound.
