Caught in, under or between a moving and a stationary object

The mechanic was prying the top of the LR suspension out of its pinned position. The suspension was supported by a come-a-long near the bottom of the barrel. As the weight began to shift, it caused the prybar to recoil rapidly, pinching/severing the tip of his right pinky finger.

Fall to the walkway or working surface

He was on the ground walking backwards pulling a cable off the reel truck when he stumbled and fell backwards onto his right hip. The fall resulted in a fractured right hip.

Over-exertion in lifting objects

Individual was stocking parts in the warehouse when he felt pain in his lower left side. Individual was taken to see a doctor, had a surgical consult on February 9th, and underwent surgery on February 10th for an inguinal hernia.

Struck by falling object

Employee was jacking a clutch on the propel using a port-a-power, the ram kicked out and hit him in the R.H. leg.

Struck against a moving object

Employee was returning to regrade area with a loaded 657G Scraper to dump his load. As he entered the dump area he hit a bump that bounced him up against the seatbelt. As his body returned to the seat the scraper was bouncing up causing severe back pain.

Bodily reaction, (Not Elsewhere Classified)

While working on a track dozer, the technician stood up from a squatting position on the concrete shop floor and turned to his left. The twisting motion fractured the fibula just above the right ankle.

Struck against stationary object

The haul truck operator stepped on a chunk of material, rolling the right ankle over, fracturing the ankle while performing truck inspections at the ready line.

Fall from machine

After connecting a battery charger, the mechanic turned towards the cab when the heel of his boot caught on the deck plate causing his foot/ankle to roll over and off a 4"" step, resulting in a fracture to his foot.

Fall to lower level, (Not Elsewhere Classified)

Employee was walking from a locomotive to the truck when he fell into a hole that was weed covered twisting his ankle. The employee went to the emergency room on 01/02/2009 and had an xray showing an Avulsion fracture to the left ankle.

Individual was helping feed a hose over the fan motor. Once in place, the hose slipped and the hose fitting pinched his finger, requiring stitches.

Bodily reaction, (Not Elsewhere Classified)

EE was kneeling down removing a guard from the side frame on a shovel. When the EE stood up and started walking, a pain went through his left knee at that point. He did not trip or slip on any foreign objects.

Technician was loosening fitting on engine when it suddenly came loose, pinching and cutting his hand between the wrench and another fitting. Cut required stitches to close.

The technician's right ring finger was pinched and lacerated (requiring sutures) between a brake assembly and a cabinet on the service truck while it was being hoisted onto the dragline.

Flash burns (welding)

A plant millwright received a welding flash burn to the eyes, due to the arc reflecting off the walls of the chute he was working in.

The mechanics right thumb was laceratd and sutured after the work table attached to his tool chest fell down pinching his thumb, as it was being pulled across the shop floor.

A mechanic slipped while using a knife to cut a wire-tie and cut his left hand. It required stitches to repair.

Fall to the walkway or working surface

Security Officer was at post in the guard shack seated in a chair. Officer fell striking the top of her head. Officer received 3 stitches in her head.

Struck by flying object

4/11 @ 2 AM, Felt irritation in eye while air arching in rev frame of shovel, sparks coming over & around head. Welding helmet & safety glasses being worn. Washed eye on site, completed shift & next shift. 4/12: 5 PM, after sleeping, eye felt irritated. Imbedded metal partical removed from right cornea by doctor using brush & spud. Returned to work next day after follow-up exam.

While working on removing an injector, the pry bar slipped and hit the individual in the upper lip, requiring stitches.

Absorption of radiations, caustics, toxic and noxious substances

Individual was removing a hydraulic line from a hoist cylinder. While removing the line, he was sprayed in the face with hydraulic fluid. He saw the doctor and was given prescription eye drops.

Caught in, under or between a moving and a stationary object

Employee had placed a light set inside the car body of a shovel and began to exit. As he climbed out, he ""hit"" the light cord pulling the light set over causing it to crash down pinching his right long finger between it and the car body.
