Struck against stationary object

Employee dropped water bottle in Cat D30 Haul unit air condition vent hole, while retrieving the bottle, employee injuried right ring finger tip when finger made contact with blower fan. While getting first aid treatment in the office, the employee fainted and broke his front tooth when he hit the desk.

Over-exertion (Not Elsewhere Classified)

Employee hurt left knee while placing Boring Bar into hole on an Excavator.

Caught in, under or between a moving and a stationary object

While checking front windshield wiper, the wiper became stuck, emplyee got out and found a 1.5 inch diameter rock stuck at the base of the wiper, he grabbed the rock with his left index finger and thumb and pulled back on it, the rock and the wiper started to move back to the start position, catching the left thumb between the wiper arm assembly, caused a fracture on left thumb.

Fall onto or against objects

Employee was replacing srceens on 5'x 20'shaker, slipped and fell and struck head on chute. Employee was examined by doctor and released to full duty. Employee was perscribed some medication for pain.
