Caught in, under or between collapsing material or buildings

The decedent, an o/operator, was located behind the truck, behind the raised dump trailer when a load of sand dumped from the trailer. The decedent was buried in the sand. Mgmt became aware of the missing driver at approx. 10:45 am and promptly began a search. EEs began digging in the pile of sand behind the decedent's truck and the decedent was discovered at approx 11:43 am.

Over-exertion in pulling or pushing objects

Employee experienced shoulder pain while opening and closing a window in the office. Employee was sitting down while opening the window, overextending shoulders. The pain progressed throughout the week. Employee requested to go to the doctor on 12/18/2017.

Over-exertion (Not Elsewhere Classified)

Employee was attempting to close a door on the water truck. The door would not close. Employee tried using excessive force while attempting to close the door. EE hit elbow on the structure of the truck and twisted left hand in the door handle.

Fall from machine

Employee was attempting to enter the D7 dozer in the pit. As the operator reached for the top hand rail, slipped and fell backwards, landing on the ground. Employee suffered small contusions on back and right arm. Doctors put employee on restrictive duty.

Employee finger got caught in the step grating after exiting the service truck causing pain on the right ring finger.
