Over-exertion (Not Elsewhere Classified)

Employee was on the secondary shaker removing the middle screen of the top deck. The employee was repositioning himself to allow the screen to be pulled higher on the deck, as he stepped and turned his right foot got caught on the upper screen causing his right knee to twist. Employee was released to restricted duty and get rechecked by doctor on 7/25/2008.

Struck by falling object

Removing a front-axle of a haul truck and was struck on the right arm when employee cut through the pivot pin. Fractured right forearm.

employee was swiping dirt off of trailer ramp with his right hand and a sliver of metal cut through his glove and little finger. Employee received stiches and was released to light duty. Employee will have stiches removed in 10 days.

Fall from machine

Employee was climbing onto CAT 230 KW Generator to turn unit on for the day when his right foot slipped causing him to fall and hurt his right knee. Employee was examined by a doctor on 1/2/09 and released to regular duty.

Over-exertion (Not Elsewhere Classified)

Employee walking to get into service truck, stepped into a low uneven ground area, felt pain in R knee. Employee was able to grab hold of service truck door to keep from falling.

Absorption of radiations, caustics, toxic and noxious substances

Employee was instructed to re-install a tire and hub assembley on a Euclid B-30 Belly Dump. While employee was sliding chain under the tire, a snake bit him on the right hand, employee was taken to the hospital and was kept under observation for six hours and released with no restrictions.
