Struck by rolling or sliding object

9/13/06, employee was transporting a piece of PVC pipe on freight elevator. As elevator moved, the pipe slid or rolled into the his left calf and he received a laceration. He treated himself, did not miss any work. On 9/20/06, he told leadman that it was getting infected and he needed to see a Dr. He was treated by Dr on 9/22/06, Dr. has not released him to RTW until 10/02/06.

Fall onto or against objects

Employee was inspecting bulk truck and shoe got caught on latch. He fell and hit knee.

Coming down stairs, he slipped and hurt left ankle.
