Caught in, under or between a moving and a stationary object

EE was helping clean waste bin, while using a swing hammer to hit the bin it came back sideways and smashed right pinky finger against the handrail. Finger is fractured. Employee is on restricted work duty.

Caught in, under or between running or meshing objects

Employee was inspecting bearing noise on pre-stretch roller when ring finger on left hand was caught by roller & mashed the end of finger & tore the skin. Finger required sutures. Employee is on restricted work duty.

Over-exertion in lifting objects

The employee strained back when lifting up 80# bags.
