Fall from scaffolds, walkways, platforms

Employee climbed up on a three and half foot high concrete slab and was cleaning salt off a conveyor belt, when he got through he jumped off the slab and twisted his right knee. On 2/10/2016 employee received a cortisone shot for Pain.

Struck against stationary object

Employee climbed up #1 992 loader to check pin on lift cylinder. He was squatting down trying to see the pin on the lift cylinder. As he raised and turned he hit his head on the mirror bracket, his hardhat and phone wound up on the ground. He had a laceration on left forehead that required 10 stiches at ER.

Struck by falling object

In the maintenance parts warehouse near the east door. EE was accessing a piece of pvc conduit near the wall, when he bumped A 30# fire extinguisher with his hardhat causing it to fall and hit his left forearm. He was taken to ER and received staples to close a four inch laceration
