Inundation of pit/quarry and operations. There are no injuries.

Employee was crawling out of the S-6 screen at secondary plant and his left knee popped out. Employee has a pre-existing left knee condition. Employee received a sprain and is off work.

Employee was getting out of the manlift after installing a hopper plate. As employee was exiting the manlift, he lost footing and twisted his right ankle. Employee received trauma and pain to his right ankle. Employee was restricted.

Employee was spraying underneath the roll crusher. The hose started slipping from his hand. Employee readjusted his weight to get a better stand. As employee went back and then forward, he twisted his right knee causing him to go back and forward again and then falling. Employee received a strain to his right knee. Employee was given a brace and was restricted.

Emp. driving to shop to get parts. Getting out of truck, got dizzy, couldn't breathe, fell out of truck, couldn't get to his inhaler. Was found on ground, ambulance was called, taken to hospital. Having asthma attack, because he couldn't get to inhaler, had a panic attack. No injury from fall. Asthma attack not work related. Document to follow.
