Absorption of radiations, caustics, toxic and noxious substances

Miner was outside dryer assisting with cleaning out of lower plenum, unknowingly standing in hot sand causing a minor burn to the side of EE's right foot. First Aid incident until doctor visit on 6-28-21 where EE received restricted duty.

Caught in, under or between a moving and a stationary object

The IE is a security guard. EE lowered a chain across a roadway, drove forward a few feet with EE's left leg outside the vehicle and the leg jammed into the ground causing EE's left knee to be injured with a break of a bone in the knee.

Struck against stationary object

In the process of loading a railcar, the affected employee received a laceration on right hand due to inadvertent contact with the loadout spout. The laceration required 5 stitches to close which prevented the employee from using that hand for the employee's job.

Dust from is controlled by a Torrit dry filter. The dry filter blower motor was locked out and guards and belts removed to service bearings. Hazards of belt replacement were discussed among employees prior to task. During belt replacement, employee caught finger between belt and sheave. Right hand ring finger tip was fractured and lacerated requiring medical treatment.

Fall from ladders

EE was cutting down 6"" pipe with torch while standing on the 2nd rung of a ladder. His retractable lanyart line was under pipe when he cut the pipe free the pipe pulled EE off of the ladder making him fall to the floor.

Struck against stationary object

Employee was removing heat tape from tall oil pipe. Employee's rib cage was leaning against the handrail. When employee reached over the handrail to pull the heat tape off, pressure was exerted on his right rib cage resulting in fracture.

Fall from scaffolds, walkways, platforms

EE WALKED OUT ONTO THE WET TRANSFER CONVEYOR CATWALK TO INSPECT #5 CONVEYOR. THE CATWALK GAVE WAY & THE EE DROPPED 20FT TO THE DRYER ROOM FLOOR.

Caught in, under or between a moving and a stationary object

EE WAS ATTEMPTING TO OPEN BOTTOM SLIDEGATE ON RAILCAR WITH A PRYBAR. THE PRYBAR GAVE WAY THE EE FELL FORWARD, CAUSING HIS LEFT HAND TO BECOME WEDGED BETWEEN THE PRYBAR & THE RAILROAD TRACK. THE FIFTH FINGER ON THE EE'S LEFT HAND WAS LACERATED & REQUIRED 10 STITCHES.

Fall from machine

AT 8:00 AM EMPLOYEE DROVE TO QUARRY TO CHECK ENGINE OIL ON #47 988B FRONT END LOADER. EMPLOYEE CLIMBED THE LADDER TO CHECK OIL. WHILE DESCENDING THE LADDER BROKE, CAUSING THE EMPLOYEE TO FALL, STRIKING HIS LEFT SIDE AGAINST THE LADDER FRAME.
