Over-exertion in lifting objects

During the installation of a heavy duty clutch in one of our on road delivery Dump Truck's, the clutch was picked up by hand, it slipped and the shifting weight caused the employee to strain his back.

Over-exertion (Not Elsewhere Classified)

Stepping over railing from rock barge to dredge pontoon left leg caught an electrical conduit causing leg to pop at knee. Dr. diagnosis left calf strain, released to modified duty/regular hours, follow-up check in one week.

Caught in, under or between a moving and a stationary object

While performing the task, flipping blow bars in the Hazmag crusher, the first blow bar removed pinned the employee's right hand, crushing the thumb. X-rays showed no broken bones.

Fall from machine

After preforming repairs & climbing down from cat walk to track of dragline, left foot contacted between pads causing loss of balance & fall backwards to ground, landing on right arm. Took to Med center for x-ray's, no broken bones. Jammed wrist, Dr. released to full duty's.
