OPERATOR DID NOT SEE THE VICTIM ON FOOT BEHIND HIM WHILE BACKING DOWN RAMP LEADING TO FEED THE HOPPER. THERE WAS ONE VICTIM INVOLVED IN THE ACCIDENT.

While putting brakes on a trailer, employee was using a tool & it slipped & hit his middle finger on left hand. His fingernail was torn loose. He was taken to an urgent care center. There was no medical treatment done on his finger at this time & he was referred to go to a specialist on 08/31/09. They removed the fingernail & he was released with no restrictions.

Caught in, under or between a moving and a stationary object

While working to clear a clogged input hood of a Hazmag crusher the side door was being opened, once the door was over center the two miners opening the door could not hold the weight of the door and material. The door then swung down crushing the first and second fingers of one of the miner's right hand between the door and body of the crusher.

While removing the guard on the tail pulley of a floating conveyor, the guard pinched the third finger, causing fracture and traumatic avulsion.

Struck by falling object

While removing a drive shaft it fell and smashed the tip of the right middle finger. The tip of the finger is fractured requiring splinting, the resulting laceration required six sutures, & a sub vascular hematoma required drilling of the nail to relieve pressure.

Struck against stationary object

Hit left knee on metal structure when entering the dredge to start his shift. At time of incident the Med center told him it was only strained. On 10/22/2008 employee received surgery to repair a torn Meniscus Ligament which was a result of the incident back on 08/04/2008. Post surgery will result in lost time and modified duty.

WHILE CLIMBING UP THE 2 STEPS TO ENTER HIS TRACTOR TRAILER HE SLIPPED ON THE TOP STEP AND STARTED TO FALL OFF. HE GRABBED THE HANDRAIL ON THE OUTSIDE OF THE TRUCK AND HELD ON WHILE HIS LEGS KICKED OUT FROM UNDER HIM THIS CAUSED A MUSCLE IN HIS LOWER BACK TO TEAR.

While performing the task, flipping blow bars in the Hazmag crusher, the first blow bar removed pinned the employee's right hand, crushing the thumb. X-rays showed no broken bones.

Over-exertion in lifting objects

During the installation of a heavy duty clutch in one of our on road delivery Dump Truck's, the clutch was picked up by hand, it slipped and the shifting weight caused the employee to strain his back.

Stepping over railing from rock barge to dredge pontoon left leg caught an electrical conduit causing leg to pop at knee. Dr. diagnosis left calf strain, released to modified duty/regular hours, follow-up check in one week.

Fall from machine

After preforming repairs & climbing down from cat walk to track of dragline, left foot contacted between pads causing loss of balance & fall backwards to ground, landing on right arm. Took to Med center for x-ray's, no broken bones. Jammed wrist, Dr. released to full duty's.

CONVEYOR BELT WAS STARTED TO TRACK BELT AND INSPECT NEWLY INSTALLED MOLD BOARD RUBBERS. MINER REMOVED TAIL PULLEY GUARD AND LOOSE CLOTHES (GLOVES) SUCKED ARM INTO PINCH POINT, AREA BETWEEN CON VEYOR BELT AND MOLD BOARD.

WHILE USING A PRY BAR TO POKE TRENCH FROZEN GRAVEL IN HOPPER. WORKER RECEIVED SEVERE BACK PAIN.
