Over-exertion in lifting objects

On Sept. 24, 2007 EE stained his back when he moved a bearing assembly he was hooking a chain hoist up to as it was lying in a pick-up truck bed. He started missing work on March 3, 2008.

Over-exertion in pulling or pushing objects

Employee was changing out a sieve box and as he was pulling on it he felt pain in his lower back. Due to complication this employee started missing work on August 13, 2007.

Caught in, under or between (Not Elsewhere Classified)

Employee was putting belts on a cyclone pump sheave wheel when he got the tip of his thumb on the right hand between the belt and the sheave causing a laceration that took sutures to close.
