An electrical motor, wrapped in plastic, was lying on the tailgate of a pick up truck. The top of the plastic was cut and as he lifted the motor to get the plastic from underneath the motor he felt pain in his back. Due to pain in his back he started missing work on 8-23-2012.

Driver stated while sitting in the truck and dumping a load of coal, he had his arm rested on the window of the tractor. The dump trailer was mid way up when the 2nd stage locking ring broke and allowed the trailer to fall. This caused a jolt to the truck which bruised the driver around and under his shoulder. Damage to trailer but not to truck.
Employee was rocking dusting by hand in the number 4 left break and as he was spreading dust his back started hurting.
Employee was bolting top in the face of the number 6 entry. When he deactivated the stab jack the canopy hit him on top of the head. He is complaining of neck and chest pain. He returned to work on his next regular scheduled shift but to complications he started missing worn on 4-18-2012.
Employee was loading cap wedges in the stockpile area onto a forklift when he tripped over a metal band that held the cap wedges together for shipment to the mine. He fell catching his fall with his right hand and knee. On 6-29-12 he was put on restricted duty due to his wrist hurting and returned to full work duty on 7-9-2012--a total of six days.
Employee was tramming a scoop in a slanted area and it slid into the rib. He complained of his back and neck hurting. He was put on restricted duty for two days and returned to his regular job.
