While in the process of getting out of car, He twisted his back.

While walking belt during examination, her boot got caught underneath rock, causing her to fall on knees. ** started missing 10/22/09**

He was carrrying 2 -5 gallon cans of oil, when he felt pain in right knee.** started missing on 05/21/10**

He was lifting feeder cable to the rib, when he pulled muscles in his back. **Started missing on 10-07-09**

He was carrying a crib block, when he tripped over a rock and fell, landing on wrist.**started missing 09-22-09**

He was starting to drill hole with bent steel, causing him to jar shoulder **started missing on 08/31/09**

while installing roof bolt, a piece of material fell infront of ATRS, falling backwards, striking him on upper thigh, (causing a tear in a ligament in right knee).

While roof bolting, his glove got caught on pinner steel and jerked shoulder. ** started missing on 09/28/09**

HE WAS CUTTING THE BANDS ON THE STRUCTURE PILE AND THE BAND BROKE CAUSING STRUCTURE TO ROLL ON ANKLE.

She was standing on trailer, loading duster, when she twisted and felt pain in back. **Started missing on 05/30/09**

He was putting up roof bolt, when a piece of material struck his right knee. Started missing on 06/12/09.

He was riding on a golf cart, when it started to slide, causing the cart to run into a rib and turn over.

HE WAS REACHING FOR A PIN, WHEN HE FELT SOMETHING POP IN HIS SHOULDER **STARTED MISSING ON 05/12/09**

while holding a feeder cable for another mechanic while he cut it with belt knife, belt knife slipped and cut the knuckle on his left hand , / he had to have 3 stitches.

He was cleaning rock off top of miner, before going into next cut, when a piece of material fell between roof bolts (4''x 8''x 1'') hitting him on right forearm causing a cut. He received 4 stitches on right forearm.

He was installing roof bolt with a pin board attached, when pushing bolt to top, he caught his right index finger between pin board and top. ( Needing stitches on end of right index finger.)

While cutting on belt, knife slipped and cut his right leg. He had 9 stitches to close wound.

He was cutting conveyor belt with a belt knife, when he accidently cut left forearm , needing two stitches.

HE WAS STRIKING HAMMER AGAINST ANOTHER HAMMER, METAL FELL INTO ARM.

WHILE CLEANING WATER SPRAYS ON FEEDER, HE HIT A SHARP PIECE OF METAL WHICH CUT HIS RT. FINGER.

While prying down draw rock, a piece of rock fell and hit forearm.

HE WAS WALKING AROUND ROOF BOLTER, SLIPPED AND CAUGHT SELF WITH HAND.
