He was cleaning rock off top of miner, before going into next cut, when a piece of material fell between roof bolts (4''x 8''x 1'') hitting him on right forearm causing a cut. He received 4 stitches on right forearm.

He was installing roof bolt with a pin board attached, when pushing bolt to top, he caught his right index finger between pin board and top. ( Needing stitches on end of right index finger.)
