While installing a roof bolt, he tripped and fell against the atrs. (straining his shoulder) **started missing 03/29/2016**

Employee bent over to pick up timber, felt pain in lower back. Possible surgery (***Started missing 11/07/2016 )

Struck by falling object

He was putting strap on miner cable, while bent over, a piece of material fell and struck him on lower back, causing him to hyper extend his knee.

He was installing roof bolts in face. He went to retrieve another bolt, when a piece of coal fell out. Striking him in back of leg and ankle. (He received stitches on ankle.)

He was riding on golf cart. They back off road into x-cut, when a piece of material fell out and struck him on back, neck. **(He began missing work on 9-8-15)**

Over-exertion in lifting objects

He was hanging high voltage cable, when he felt pain in his right shoulder. (an tear in his rotator cuff) ** started missing on 01/27/2016**

Struck by falling object

He was standing next to a stack of overcast panels. They started to pull a panel off top of stack. The panel slid off top of stack, swung around and stuck him across legs. (fracture)

Fall to the walkway or working surface

HE was replacing a joint fitting to waterline, when he turned to get parts off mantrip. He slipped and fell back , and landed on right elbow, which jammed shoulder. * started missing on 05/12/2015 with tendon tear*

Struck by falling object

Drilling hole to install roof bolt, Material fell out, slid off canopy, striking him on left ankle. (chipped a bone )

Fall onto or against objects

While cutting a bit lug off on the head of the miner a piece of loose material fell out and struck the mechanic in the back. The mechanic then fell forward and struck the head of the miner. ( Received stitches )

Struck against stationary object

He is 6 ft. 5 inches tall walking down a entry that is 5 ft. 6 inches tall. While walking he had his head tilted to one side, when he hit his ear on the end of a roof bolt. (he received stitches)

Struck by falling object

While moving the miner cable, a piece of rock fell and struck him on left thumb, (received stitches)

Caught in, under or between a moving and a stationary object

EE was helping replace a worn out screen, when installing the new screen, the pan raised up and smashed his finger between pan and I-Beam.

Struck against a moving object

While driving inby on a 4-man diesel and looking in towards the belt, hit hard hat on the roof. Causing lacerations to head, received stitches.

He was driving down roadway, when a tractor ahead of him snagged the lifeline. It stretched, broke and the direction cone on lifeline struck him on cheek. (He received stitches)

Caught in, under or between a moving and a stationary object

He was moving a section of chute work on a cart. He had his finger on the outside of the chute. He pinched it between chute and impact box. (He received stitches)

Struck by falling object

He was on his knees, building a crib. when a piece of rock fell out, hit his shoulder and slid off striking his hand which was on ground. ( received stitches to hand)

Fall to the walkway or working surface

HE was using a thread tapping tool. He put pressure on the wrench and the tool broke, causing him to fall forward , cutting his index finger on the tool.( he received stitches.)

Struck by falling object

He was working on shuttle car. He had lifted the cover to work in panel. He wedged cover in hog wire to hold it up. The cover fell and struck him on index and middle finger (Fracture of Index finger and cut on Middle finger)

Struck by flying object

He was installing a 84"" roof bolt, he went to bend to bolt ,slipped and bolt fell out of hole and struck him on forehead . (received stitches)
