Employee received a contusion to left knee after being struck by fly rock during a blasting incident on 08/11/15. While moving behind high blocks during the blast the employee strained his lower back.

Mechanic was connecting a conveyor to a screener. He was on the conveyor when it started up and he fell off.

Standing in yard, called mechanic stating he thought he was having a heart attack. EMS summined, stopped breathing, revived in ambulance. Stopped again enroute to hospitl. Hospital attempts to revive unsuccessful. Pronounced dead of cardiac arrest. Submitted - Pending MSHA chargability determination.

Reported right shoulder, arm and elbow hurt too much to work. Hurt previously, he continued to work; not disabling. On vacation difficult to move. Off from work, will receive therapy. Recalled 12/2/2005 at 07:00, frosty morning, climbing a fixed ladder at cone, slipped, grabbing on and held with his arms. He pulled himself up and he continued to work.
