The employee was climbing down the ladder of the water truck after he checked to see if the pump was pumping water into the truck. As the employee was climbing down, he slipped off the rung, due to grease on his boots and partially cut himself before falling 8 feet to the ground. The employee bruised his back when he fell and landed on his back on the ground.

The employee was cranking up the tarp to the dump truck by hand when the handle slipped out of his hand. Due to the spring loaded nature of the tarp the handle spun quickly and hit the palm of his left hand causing a laceration.

Employee was unhooking the hose from the water pump & was hitting the hose to loosen it from the pump. As he did this he slipped on snow which caused him to slip backwards. As he tried to catch his balance he grabbed at the hose and pulled it towards his face. The hose clamps struck his nose and cheek, causing a small laceration that needed to be stitched.
