Employee was putting part of the crusher back together when his hand slipped and his finger scraped across an edge of a bolt causing a laceration.
The employee was climbing down the ladder of the water truck after he checked to see if the pump was pumping water into the truck. As the employee was climbing down, he slipped off the rung, due to grease on his boots and partially cut himself before falling 8 feet to the ground. The employee bruised his back when he fell and landed on his back on the ground.

The employee was moving a 3"" water hose when he slipped on an icy rock causing him to fall striking his left shin against another rock resulting in a contusion to his left shin.
The employee was cranking up the tarp to the dump truck by hand when the handle slipped out of his hand. Due to the spring loaded nature of the tarp the handle spun quickly and hit the palm of his left hand causing a laceration.
Employee was power washing the excavator when he slipped on the track and hit his leg against the excavator, causing a bruise to his left leg.
Employee received a hearing test on 03/21/07 and had a standard threshold shift in his hearing.

Employee was using a ratchet to loosen the nuts on the crusher cone when the ratchet snapped in half causing the employee to strike his hand against the cone causing a contusion to right wrist.
Employee was unhooking the hose from the water pump & was hitting the hose to loosen it from the pump. As he did this he slipped on snow which caused him to slip backwards. As he tried to catch his balance he grabbed at the hose and pulled it towards his face. The hose clamps struck his nose and cheek, causing a small laceration that needed to be stitched.
