Employee was moving miner cable and injured EE's back.

Employee was pulling miner cable slack. A shuttle car came around the corner catching EE's left ankle between the shuttle car and a small rock.

Employee was beating on a bolt with a hammer. A sliver of the hammer head came off and went into the index finger of his right hand.

Employee was walking up steps into the security office in the rain. She is uncertain if she slipped or tripped but she fell forward landing face down striking her nose on the steps resulting in a fractured nose. Doctor released her to return to work the same day with no restrictions. She reported she was using the hand rail at the time of the incident.

Employee was loading supplies onto roof bolter and his knee popped.

Pinner operator was putting up a roof bolt. His hand slipped from chuck and got caught between chuck & head of bolt. Hand was cut between thumb and first finger.

Pinner operator was coupling steel together when his glove got caught between steel, cutting his left index finger.

EMPLOYEE WAS REMOVING A ROCK FROM BETWEEN NON-MOVING BELTS AND STRAINED HIS BACK.

Employee had gloves on while removing wiring from around the roof bolt plates. A piece of metal from roof bolt plates went thru his glove & into his finger.

EE WAS DRILLING THROUGH SOME CAP COAL WHEN THE COAL FELL LOOSE AND HIT HIM ON THE SHOULDER AND BACK.

EMPLOYEE WAS TRYING TO MOVE COAL AND ROCK ON THE BELT DRIVE WITH A SLATE BAR. THE BAR GOT CAUGHT IN THE DRIVE AND PUSHED AGAINST EE'S CHEST.

EE WAS TRAINING BELT LINE, BOTTOM ROLLER DROPPED AND FELL ON LEFT HAND.

EMPLOYEE WAS RESTOCKING MINER CABLE USING A CRUMPING TOOL, WHEN A PIECE OF METAL BROKE OFF AND WENT INTO LEFT HAND.

EE WAS OPERATING REMOTE MINER, THEN HE MOVED THE MINER CABLE OUT OF THE WAY ABOUT 30 FEET, AND HURT LOWER BACK.

EMPLOYEE WAS WORKING ON POWER BOX WHEN SCOOP DIPPER DROPPED OFF CAMBER, HITTING HIS LEFT THUMB.

DRILLING HOLE IN MINE ROOF FOR SURVEY POINT USING HAND CRANK DRILL STRAINED GROIN. MINE HEIGHT WAS 36" AND PERSON WAS PUTTING UPWARD PRESSURE ON DRILL WITH UPPER THIGH TO DRILL HOLE.

EE WAS TRAMMING MINER, THE MINER HEAD WENT OVER A DIP AND CAUSED THE EE TO HIT HIS HEAD ON THE TOP.

EMPLOYEE WAS RIDING THE MANTRIP OUTSIDE WHEN HE HIT HIS HEAD ON OVERCAST.

EMPLOYEE WAS RIDING THE DIPPER OF THE SCOOP TO HIS SECTION AND THE DIPPER JARRED HIM.

EMPLOYEE WAS WORKING ON A PORTAL BUS, KNOCKING ABOLT OUT WHEN HE HIT HIS LITTLE LEFT FINGER WITHHAMMER. FINGER ON RIGHT HAND.

EMP WAS OPERATING SHUTTLE CAR WHEN HE RAN OVER ASMALL ROCK IN HAULAGE WAY CAUSING HIM TO STRIKE HIS HEAD AGAINST ROOF.

EE WAS WORKING WITH OTHER. OTHER WAS DRIVING THE SCOOP AND HE WAS IN THE SCOOP DIPPER. THEY WERE BACKING UP AND OTHER STOPPED THE SCOOP TO CHECK SOMETHING WHEN EE S HEAD GOT CAUGHT ON CABLE.

MAN WAS CLEANIN1 BELT WITH SHOVEL IN LOW TOP WHEN A BELT SPLICE CAUGHT THE SHOVEL THROWING IT BACK INTO HIS FACE CUTTING HIS FOREHEAD AND BRIDGE OF HIS NOSE

EMP HAD CAME OU7 SIDE TO SERVICE TRACTOR ABOUT 1 PM & HAD BEEN INSTRUCTED TO HELP OUTSIDE MAN UNTIL QUITING TIME 2:30 PM. HE WAS BENT OVER USING A PORTABLE GRINDER ON TAIL PIECE, WHEN AT APPRO X 2:20 PM ANOTH9R TRACTOR OPERATOR CAME OUT OF MINE & COMING TOO CLOSE TO MEN WORKING ON TAILPIECE MASHED MANS LEFT ANKLE BETWEEN TAILPIECE & TRACTOR

EMPLOYEE WAS ATTEMPTING TO PULL DOWN A PIECE OF DRAW ROCK WHICH FELL AND STRUCK HIM ON THE LEFT THIGH CAUSING A BRUISED MUSCLE
