Showing all 25Employee was pulling miner cable slack. A shuttle car came around the corner catching EE's left ankle between the shuttle car and a small rock.
Employee was beating on a bolt with a hammer. A sliver of the hammer head came off and went into the index finger of his right hand.
Employee was walking up steps into the security office in the rain. She is uncertain if she slipped or tripped but she fell forward landing face down striking her nose on the steps resulting in a fractured nose. Doctor released her to return to work the same day with no restrictions. She reported she was using the hand rail at the time of the incident.
Employee was loading supplies onto roof bolter and his knee popped.

EMPLOYEE WAS REMOVING A ROCK FROM BETWEEN NON-MOVING BELTS AND STRAINED HIS BACK.
Employee had gloves on while removing wiring from around the roof bolt plates. A piece of metal from roof bolt plates went thru his glove & into his finger.
EE WAS DRILLING THROUGH SOME CAP COAL WHEN THE COAL FELL LOOSE AND HIT HIM ON THE SHOULDER AND BACK.
DUE TO WEATHER CONDITIONS METHANE WOULD NOT RAISE FROM BOX CUT AND METHANE RECIRCULATED IN MINE TO 1%.

EE WAS OPERATING REMOTE MINER, THEN HE MOVED THE MINER CABLE OUT OF THE WAY ABOUT 30 FEET, AND HURT LOWER BACK.
EMPLOYEE WAS WORKING ON POWER BOX WHEN SCOOP DIPPER DROPPED OFF CAMBER, HITTING HIS LEFT THUMB.
DRILLING HOLE IN MINE ROOF FOR SURVEY POINT USING HAND CRANK DRILL STRAINED GROIN. MINE HEIGHT WAS 36" AND PERSON WAS PUTTING UPWARD PRESSURE ON DRILL WITH UPPER THIGH TO DRILL HOLE.
EE WAS TRAMMING MINER, THE MINER HEAD WENT OVER A DIP AND CAUSED THE EE TO HIT HIS HEAD ON THE TOP.
