A piece of draw rock measuring 0"-2" X 24" X24" fell on canopy and slid off hitting subjects left leg and foot, fracturing left tibia about 6 inches above ankle.
Subject was trying to straighten roof bolt. Canopy struck subject in head. Stab jack was set on loose bottom. Bottom busted and canopy struck subject in head. Head aches. Waiting on test results.

The continuous miner operator was tramming across the working section, tripped and fell causing him to lose control of the machine pinning him against the coal rib resulting in a fracture of the top & bottom of pelvis (left).

EE WAS HELPING TO MOVE THE CONTINUOUS MINER CABLE AND COAL RIB ROLLED OFF, STRIKING HIS BACK JUST ABOVE THE WAIST LINE.

THE INJURED PERSON WAS HARD LOADING A BOX OF GLUE. WHEN HE TURNED TO LAY IT ON HIS PINNER, HE FELT A SHARP PAIN IN HIS BACK.
EE WAS OPERATING OFF STANDARD 21 SC SHUTTLE CAR. HAD COMPLETED BEING LOADED, WAS CHANGING SEATS, SLIPPED AND FELL. SHUTTLE CAR PINNED (R0LLED)EE AGAINST COAL RIB.

UNKNOWN THIS IS AN ALLEGED OCCURANCE THAT SUPPOSEDLY HAPPENED 3-14-95. OPERATOR WASNT NOTIFIED UNTIL 4-7-95 230 PM.

REPAIR MAN TURNED CIRCUIT BREAKER TOT HE ON POSITION AND AT THE SAME TIME ACCIDENTLY HIT HTE SPPEED CONTROLLER CAUSING THE #1BATT OPERATOR SUPPLY TRACK MOTOR TO KNOCK THE VICTIM DOWN BETWEEN T HE #1 TRAK MOTOR AND THE #2 PERSONNEL TRACK CAR. CAUSING KNOT ON HEAD AND LOWER BACK INJURY.
WAS HELPING PULL MINER CABLE OUT OF ROADWAY. COMPLAINED OF LOWER BACK PAIN.
WAS LIFTING DRILL POT [ROOF BOLTING MACHINE] LOADING ONTO TOOL CAR. COMPLINED OF LOWER BACK HURTING.
EMPLOYEE WAS UNLOADING HYDRAULIC CYLINDER FROM SUPPLY MOTOR (TRACK). HYDRAULIC CYLINDER CAUGHT EMPLOYEES LEFT RING FINGER AGAINST TRACK MOTOR.BROKEN LEFT RING FINGER AT FIRST JOINT (TIP) AND M ASHED.
EMPLOYEE WAS DRILLING TEST HOLE IN ROOF NO. 2 ENTRY WITH 300 GALIS ROOF DRILL. A PIECE OF DRAW SLATE APPROX. 1-3" THICK X 2' X 4' FELL OUT BETWEEN ROOF BOLTS AND PINNED EMPLOYEE AGAINST ROOF B OLT MACHINE. BRUISED NECK, SHOULDE AND UPPER PART OF BACK.
EE WAS REMOVING LID FROM SHUTTLE CAR WHEN LID FELL AND STRUCK LEFT KNEE. THIS ACCIDENT OCCURED 8-2-93 AND EE DID NOT MISS WORK UNTIL 9-15-93.
