EE was welding at the face. The arc from the weld ignited a hole in the cutting torches burning EE's hands.

While installing a roof bolt, the roof bolt fell out of the top and struck employee in right thumb. Injury was not the result of a training or compliance issue. The incident was reported to the employer the next day.

A piece of draw rock measuring 0"-2" X 24" X24" fell on canopy and slid off hitting subjects left leg and foot, fracturing left tibia about 6 inches above ankle.

Subject was trying to straighten roof bolt. Canopy struck subject in head. Stab jack was set on loose bottom. Bottom busted and canopy struck subject in head. Head aches. Waiting on test results.

Injured back (strained) while shoveling loose coal and putting on belt conveyor.

Subject alleges that he was lifting a scoop tire trying to stand it up. He felt pain in his lower back.

Subject alleges he was hanging coal drill cable to roof bolt plate. The drill operator engaged cable reel and it struck subject in rib cage. Bruised and possible fractured ribs.

Subject (roof bolter) operator was injured by a piece of draw rock that was dislodged from the mine roof in the No. 2 entry when the ATRS was disengaged from the mine roof. Bruised back and fractured left ankle.

The continuous miner operator was tramming across the working section, tripped and fell causing him to lose control of the machine pinning him against the coal rib resulting in a fracture of the top & bottom of pelvis (left).

Welding cutter head hyd/jack 14-15 joy cont/miner. Punctured hole in hyd/jack. Oil spray on both hands and face causing 2nd and 3rd degree burns.

Operating battery powered S&S scoop. While exiting operators deck, slipped and left knee made a popping sound.

EE was setting miner up to turn cross-cut, started cutter head, when the cutter head started to cut a piece of rock and coal struck EE's left foot.

Subject was operating 10 SC Joy shuttle car. Switched direction of travel (change seats) felt pain in left hip.

Employee was plastering a stopping and got plaster in his eyes.

When driving mantrip out hit small rock with wheel making steering wheel hit his right hand. He went to hospital. After he went home he called back and said it was just bruised.

Rib coal & rock rolled out struck subject's lower back.

Subject was installing shear pin in feeder pick breaker. He injured left foot kicking reducer shaft (fracture & contusion).

Picked up bucket of miner bits (20) in one hand & miner remote box in other. Hit head on low place in mine roof, twisted head, felt sharp pain in back. Pulled & strained back.

Operator had hand lying on leveling arm when he raised the head of the bolting machine & caught his finger between leveling arms.

Installing roof bolt (resin) with 8 x 8 x 1" plate. Drill pot mashed left hand against ATRS. Fractured left thumb.

Piece of rock fell out between roof bolts, hitting subjects head and neck.

Subject was lowering the boom of the roof bolting machine and the drill pot came in contact with left knee. bruised left knee.

Subject was positioning drill pot to install roof bolt. Swung boon out with left hand elbow of right hand hit coal rib and jammed wrist against drill pot. off-side operator.

Subject was installing roof bolts. Left hand caught between roof bolt plate while spinning and the head (pot) of machine. Dislocated left small (pinky) finger.

WAS RAISING 21SC JOY SHUTTLE CAR WITH LIFTING JACK AND SLIPPED AND FELL. LIFTING JACK BAR HIT RIBS LEFT SIDE OF BODY.

EE CLAIMS THAT RIB ROLL HIT HIS RIGHT HIP. BRUISED RIGHT HIP. WHILE OPERATING EIMCO CONT. MINER.REMOTE CONTROLLED.

EE WAS LIFTING CRIB BLOCKS. HE SAID HE FELT A PULL IN HIS LEFT ABDOMEN AREA. THIS ACCIDENT HAPPENED ON 2/22/03. HAD NO LOST TIME UNTIL HE WAS OPERATED ON 4/16/03 FOR DOUBLE HERNIA.

SUBJECT WAS TRAMMING DDO-13 FLETCHER ROOF BOLTER, CROSSING CONT MINER CABLE HE WAS TRAMMING IN REAR DECK WHEN HE CROSSED CABLE THREW REAR DECK WAS NOT IN LOW POSITION IN TO MINE ROOF. RIGHT UP PER ARM AND SHOULDER HIT ROOF.

SUBJECT WAS OPERATING 21SC SHUTTLE CAR ON 002 SECTION, RAN OVER A PIECE OF ROCK IN ROADWAY. ALLEGES THAT IT SHOOK HIM CAUSING PAIN TO BACK.

SUBJECT WAS OPERATING REMOTE CONTROLLED CONT/M WER PIECE OF DRAW ROCK FELL FROM ROOF HIT HIS RT HAND.

UNKNOWN, EE SAID HE DID NOT KNOW HOW OR WHEN HE HURT HIS BACK.

SUBJECT WAS BOLTING ROOF, AND A SMALL PIECE OF DRAW ROCK STRUCK HIM ON LEFT SHOULDER.

A PIECE OF DRAW ROCK FELL & SLID DOWN SHOULDER & NECK WHILE OPERATING THE REMOTE CONTROLLED 2480-1 EIMCO CONTINUOUS MINING MACHINE.

EE WAS HELPING TO MOVE THE CONTINUOUS MINER CABLE AND COAL RIB ROLLED OFF, STRIKING HIS BACK JUST ABOVE THE WAIST LINE.

SUBJECT WAS OPERATING ROOF BOLTING MACHINE, CAUGHT IS HAND BETWEEN THE PIECES OF PINNER STEEL.

THE INJURED PERSPN SAID HE WAS PULLING ON MINER CABLE WHEN THE STRAIN HIS BACK.

THE INJURED PERSON SAID HE WAS BENDING A BOLT WHEN HE SLIPPED AND FELT PAIN IN HIS BACK.

THE INJURED PERSON WAS HARD LOADING A BOX OF GLUE. WHEN HE TURNED TO LAY IT ON HIS PINNER, HE FELT A SHARP PAIN IN HIS BACK.

EE WAS OPERATING OFF STANDARD 21 SC SHUTTLE CAR. HAD COMPLETED BEING LOADED, WAS CHANGING SEATS, SLIPPED AND FELL. SHUTTLE CAR PINNED (R0LLED)EE AGAINST COAL RIB.

OPERATING FLETCHER ROOF BOLTING MACHINE, #3 ENTRY, RT BREAK. SMALL PIECE OF DRAW ROCK, 12"X12"X 1/2" THICK FELL FROM ROOF PINNING LEFT HAND AGAINST THE MACHINE, MASHING HIS MIDDLE FINGER, RING FINGER AND PINKIE.

PIECE OF ROOF ROCK FELL OUT BETWEEN ROOF BOLT PATTERN.

EMPLOYEE WAS OPERATING 300 GALIS ROOF BOLTER INSTALLING ROOF BOLTS #2 ENTRY ROCK FELL, FROM ROOF 0" TO 5 -6" THICK X 3-4 FT. LONG AND 24" WIDE BETWEEN ATRS AND LAST PERMANENT ROOF SUPPORT STRI KIGN OPERATOR ON HIS BACK, SSPRAINING LIGIMENTS AND MUSCLES FROM NECK DOWN TO MIDDLE PART OF BACK.

UNKNOWN THIS IS AN ALLEGED OCCURANCE THAT SUPPOSEDLY HAPPENED 3-14-95. OPERATOR WASNT NOTIFIED UNTIL 4-7-95 230 PM.

EMPLOYEE WAS REACHING FOR A SLATE BAR WHICH WAS LAYING AGAINST COAL RIB. SAID IT FELT LIKE SOMETHING HIT HIM IN THE BACK. (NOTHING HIT HIM) RESUTLED IN STRAINED, SPRAINED LOWER BACK.

REPAIR MAN TURNED CIRCUIT BREAKER TOT HE ON POSITION AND AT THE SAME TIME ACCIDENTLY HIT HTE SPPEED CONTROLLER CAUSING THE #1BATT OPERATOR SUPPLY TRACK MOTOR TO KNOCK THE VICTIM DOWN BETWEEN T HE #1 TRAK MOTOR AND THE #2 PERSONNEL TRACK CAR. CAUSING KNOT ON HEAD AND LOWER BACK INJURY.

WAS HELPING PULL MINER CABLE OUT OF ROADWAY. COMPLAINED OF LOWER BACK PAIN.

WAS LIFTING DRILL POT [ROOF BOLTING MACHINE] LOADING ONTO TOOL CAR. COMPLINED OF LOWER BACK HURTING.

EMPLOYEE WAS UNLOADING HYDRAULIC CYLINDER FROM SUPPLY MOTOR (TRACK). HYDRAULIC CYLINDER CAUGHT EMPLOYEES LEFT RING FINGER AGAINST TRACK MOTOR.BROKEN LEFT RING FINGER AT FIRST JOINT (TIP) AND M ASHED.

EMPLOYEE WAS DRILLING TEST HOLE IN ROOF NO. 2 ENTRY WITH 300 GALIS ROOF DRILL. A PIECE OF DRAW SLATE APPROX. 1-3" THICK X 2' X 4' FELL OUT BETWEEN ROOF BOLTS AND PINNED EMPLOYEE AGAINST ROOF B OLT MACHINE. BRUISED NECK, SHOULDE AND UPPER PART OF BACK.

EE WAS REMOVING LID FROM SHUTTLE CAR WHEN LID FELL AND STRUCK LEFT KNEE. THIS ACCIDENT OCCURED 8-2-93 AND EE DID NOT MISS WORK UNTIL 9-15-93.
