EE was welding at the face. The arc from the weld ignited a hole in the cutting torches burning EE's hands.

Struck by falling object

While installing a roof bolt, the roof bolt fell out of the top and struck employee in right thumb. Injury was not the result of a training or compliance issue. The incident was reported to the employer the next day.

An unplanned roof fall occurred in the 2nd North West Mains, Spad # 815. The fall was appx. 40' Long 4' Thick. The fall is dangered off at all approaches.

Occupational Pneumoconiosis-Board findings-AWARD ORDER received on 4.18.2014 from AIG.EE has not missed work due to this illness. He is off work due to having knee surgery.

EE was splashed on the face with hot oil while moving it out the way.

EE WAS STEPPING OFF TAILPIECE AFTER GETTING ROCK UNSTOPPED AT THE HEAD AND TWISTED HIS LEFT KNEE. EE DIDN'T SEEK MEDICAL ATTENTION/OR MISS WORK UNTIL 2.7.2014.

EE fell to ground bruising the left elbow while working underground-finished shift.

EE was tramming 4-wheeler to section, hit a bump and hit head on mine roof.

While hanging shuttle car cable draw rock fell from between bolts and hit EE on left leg and foot. Rock size 40"" wide 10' long and 6"" thick.

EE was setting breaker up when it blew up causing burns to right side face, ear and Right hand.

EE was pulling slack from shuttle car cable to re-hang when he was shocked. PPE: EE was wearing rubber gloves.

EE was bolting top when a corner rib draw rock fell hurting his back. EE finished his shift bolting top--He went to see a Dr. on 6.21.13 to be checked-contusion on back. The EE has RTW FULL DUTIES.

EE was working on the bolter when the chain ratchet slipped dropping the scissors (arm Pot) on the left index finger causing a laceration that required sutures. EE missed one day to follow up with the doctor and has RTW modified duty until the sutures are out and laceration healed.

EE was lifting/moving a shield from the Miner then complained of his left shoulder hurting.

EE was getting back into the scoop when he put pressure on his knee it began to hurt.

EE was hanging cable in the #4 Entry-cable was pulled down, knocking him to the ground.

EE was hauling coal to feeder with shuttle car, he hit his forehead on the fly pad, knocking his head back and cutting the back of head.

Mine Foreman was backing up his 3 wheeler ride when the miner cable caught the back of his head bent into chest causing neck and left shoulder pain. NOTE: EE did not miss work or seek medical attention until 7.23.12

EE was wearing protective knee pads that began rubbing blister on the right knee causing inflammation and redness. EE worked 3 hours on the 20th, could not finish shift, he is going to seek medical attention at Logan Regional.

This Incident happened after work in the parking area. Employee was showing a hand gun to co-worker, as he was putting his hand gun back in the car the gun accidentally discharged wounding himself in the left hand. He was treated at Williamson Memorial Hospital. The WV State Police investigated the incident and filed a report.

EE was driving the 2 man carrier when he lost grip of the steering wheel and it hit his right hand. EE finished shift doing regular duties.

EE was drilling outside hole and a piece of rock 6 ft long and 8 inches thick fell and hit him on the head, neck and left shoulder. Note: EE RTW on Friday and worked his regular duties no complaints, did not inform supervisor that he went to hospital. Began missing work Monday 1.30.12

Employee was moving oxygen and acetylene tanks out of the roadway-felt a Pain in the bottom left side of stomach.

Employee was leaning forward checking for hydraulic leaks on the bolter. He was watching the hoses while moving the levers, the pot went up and caught his right arm causing a fracture.

Crew was making a power move, employee was pulling cables with a 4-wheeler when a piece of draw rock approx. 16 inches wide, 42 inches long and 2 inches thick fell out between bolts hitting the employee in the back of the neck, the coal seam in this mine is appx. 36 inches.

Employee was attempting to load belt rollers onto the ride to take underground; when he cut the bands on the pallet of the rollers they rolled off the pallet and hit his left foot and ankle.
