EE was working on the bolter when the chain ratchet slipped dropping the scissors (arm Pot) on the left index finger causing a laceration that required sutures. EE missed one day to follow up with the doctor and has RTW modified duty until the sutures are out and laceration healed.

EE was hauling coal to feeder with shuttle car, he hit his forehead on the fly pad, knocking his head back and cutting the back of head.
Mine Foreman was backing up his 3 wheeler ride when the miner cable caught the back of his head bent into chest causing neck and left shoulder pain. NOTE: EE did not miss work or seek medical attention until 7.23.12
EE was wearing protective knee pads that began rubbing blister on the right knee causing inflammation and redness. EE worked 3 hours on the 20th, could not finish shift, he is going to seek medical attention at Logan Regional.
This Incident happened after work in the parking area. Employee was showing a hand gun to co-worker, as he was putting his hand gun back in the car the gun accidentally discharged wounding himself in the left hand. He was treated at Williamson Memorial Hospital. The WV State Police investigated the incident and filed a report.
EE was standing behind manbus when the operator began to back up, hitting the EE left Foot.

Crew was making a power move, employee was pulling cables with a 4-wheeler when a piece of draw rock approx. 16 inches wide, 42 inches long and 2 inches thick fell out between bolts hitting the employee in the back of the neck, the coal seam in this mine is appx. 36 inches.

Employee was attempting to load belt rollers onto the ride to take underground; when he cut the bands on the pallet of the rollers they rolled off the pallet and hit his left foot and ankle.

Tramming bolter to another place, front end went in hole causing the back end to go into the top. Employee's neck and shoulder hit the roof top causing shoulder and neck pain.
EE was helping take shuttle car to the section. He was turning a ride around to get out of the way of the shuttle car. When turning the ride, his head jammed into the mine roof. EE stopped what he was doing and stated his neck was hurting.

EMPLOYEE'S RIGHT FOOT WAS INJURED AFTER THE SCOOP THAT HE WAS OPERATING DRIFTED AND RAN OVER IT.HE HAD EXITED THE SCOOP TO HANG A LINE CURTAIN.
EE WAS LOADING SUPPLIES FROM SCOOP BUCKET ONTO ROOF BOLTER. HE PLACED HIS HAND ON A SHARP METALSTRAP, LACERATING HIS PALM.
A ROOF FALL OCCURRED IN THE LAST OPEN BREAK FROMENTRY #3 TO #4. THE FALL MEASURED 80' LONG BY 19' WIDE BY 9-10' THICK.
EE WAS RIDING ON THE MANTRIP. THE OPERATOR BACKED UNDER A CABLE THAT WAS HUNG OVERHEAD. THE CABLE CAUGHT THE BOX CONTAINING SCSRS AND PUSHED IT OVER STRIKING EMPLOYEE.
EMPLOYEE WAS PREPARING TO SERVICE THE ROOF BOLTER, WHILE HE WAS STANDING BEHIND THE MACHINE, THE OPERATOR INADVERTANTLY AVCTIVATED THE CABLE REEL AND THE CABLE STRUCK EMPLOYEE IN THE ABDOMEN AND CHEST, KNOCKING HIM OVER.
EE WAS PULING ON A RIPPER MOTOR WHILE IN A BENT OVER POSITION.
EE PULLED THE LEVER TO LOWER THE CANOPY ON THE ROOF BOLTER. EE CLAIM THAT THE LEVER STUCK. THE CANCEL CAME .

EE WAS HANGING HI-LINE. WHILE TRYING TO STRAIGHTEN A HANGER WITH A PAIR OF CHANNEL-LOCKS, HE INJURED HIS MIDDLE FINGER ON HIS LEFT HAND. HE RECEIVED A PRESCRIPTION FOR THIS INJURY.
