WELDING LEAD WIRE HAD WORN PLACE WITH THE WIRES EXPOSED AND EE HAD WIRE UNDER HIS LEFT LEG. THE WIRE ARCED UNDER HIS LEG BURNING HIS LEG WITH 1ST AND 2ND DEGREE BURNS.

EE WAS TROUBLESHOOTING A CIRCUIT WHEN IT WAS DETERMINED THAT THE KNIVES ON A FUSIBLE DISCONNECT WERE NOT ENGAGING. THE EMPLOYEE, A QUALIFIED ELECTRICIAN, USED A SCREWDRIVER TO RESEAT THE KNIVE S. HE DROPPED THE SCREWDRIVER WHICH CONTACTED THE ENERGIZED COMPONENTS. THIS CAUSED A FIREBALL WHICH BURNED THE EMPLOYEE'S ARMS AND CHEEKS.
