Struck by falling object

While EE was cleaning material off a catwalk a rock from an overhead conveyor fell and struck them in the back of the head as EE was bending over causing a laceration. The EE received staples at a local clinic to close the wound.

While welding, a shop fan was re-positioned blowing dust in the victim's eye. The following morning, victim was in discomfort and was treated by a physician to remove a piece of rust and was issued a prescription eye drop.

Employee was washing under the supports under the KVS scrubber. Employee discovered a hole through his right boot. Puddle water got in his boot and contacted the top part of his right foot. He noticed irritation a few minutes later.

Fall from machine

Employee was on top of a pneumatic trailer, working with lid number 2, when he fell off the trailer and landed on the ground. He sustained 3 broken ribs (left rear) and a broken left thumb.

The employee struck right knee on a metal table while dismounting a forklift resulting in a laceration requiring 4 staples.

Employee was helping with kiln brick removal and in the process got lime dust in his left eye. He was wearing company supplied safety glasses at the time of the incident. He tried to flush out his eye repeatedly while still at work and afterward. His eye was still irritated several days later when he was take to the doctor and given a prescription for eye drops.

Accident was reported on 3/23/07 as a strain to lower back will driving haul truck. Employee continued to work his regular job hauling mud until 4/3/07 when he returned to work that morning at 8:00am with a doctor restrictions until the 12th of April.

Employee recieved test results of a lung abnormality in April 2006. He then went to a doctor and returned to work with restriction, we then removed him from his regular job as scaler and placed him on the crusher. We sent him to an occupational Dr. in Ft. Smith with results of no abnormality and no restrictions.

employee was replacing product screen at the kiln when he bumped a beam, while lifting the screen to another employee up above causing lime to fall on his face and in his eyes.

Raking kilns when a large piece of coal ring fell off the discharge dam hitting the rake. The side of the rake struck him on the right side of nose resulting in stitches.

Lime dust accumulated in the bend of his right arm while cleaning in penthouse. The lime activated with sweat and caused burns. The injury became infected and employee went to the doctor on 11/17/05.

Employee was plant oiler at the time of injury. He stepped out of the oiler truck twisting his left ankle on August 26 2005.He has continued his regular work duties until April 25 2006 when is Dr. removed him from all work activity for 30 days. At the end of 30 days he will be reevaluated.

Employee was operarting the skid steer cleaning out the loading floor. Quicklime on the floor was damp, steam vapors surrounded the skid steer. Operator then tried to get out of the loader and slipped. Using his left hand to break the fall, the employee's hand stuck in the quicklime.

A LICENSED BLASTER SET OFF A SHOT IN THE 25 NORTH K INTO A PILLAR. AS THE SHOT WAS IGNITED, INJURED WAS DRIVING BESIDE THE BACKSIDE OF THE PILLAR ON THE HAUL ROAD. PART OF THE SHOT BROKE THROUGH EXPOSING THE EMPLOYEE.

A SMALL ENCLOSURE SURROUNDING A BLOWER FOR A PNEUMATIC CONVEYING SYSTEM CAUGHT FIRE. THE STRUCTURE WAS A METAL SIDING, WOODEN FRAME BUILDING WITH PLYWOOD WALLS. THE SOURCE OF THE FIRE IS UNKNO WN. THE FIRE WAS PUT OUT WITH THE PLANT WATER TRUCK. THE LOCAL FIRE DEPARTMENT WAS CALLED IN. NO ONE WAS INJURED.

EE SAID HE WAS DRIVING PICKUP AROUND CORNER TO GET SAMPLE WHEN HE MET EE WHO WAS DRIVING A 769 HAUL TRUCK. HE SAID HE STOPPED BUT THE AHUL T4UCK DID NOT. THE HAUL TRUCK PUSHED THE PICKUP BACKW ARD APP 50'. WHEN THE TRUCK STOPPED, THE RIGHT FRONT WHEEL OF THE HAUL TRUCK HAD CAVED INT HE DRIVERS DOOR OF THE PICK UP.
